Responsibilities
  • Enter patient calls and messages into the electronic medical record database.
  • Ensure required patient medical records and information are available and documented.
  • Coordinate internal and external referrals and schedule patient appointments.
  • Answer phone calls and relay messages to care team members and other resources necessary for patient care.
  • Monitor provider schedules, including add-ons, cancellations, no-shows, and overbooks, and schedule testing, referral, and follow-up appointments.
  • Prioritize referrals by urgency and address them in a timely manner.
  • Send referrals to external healthcare facilities for services not provided by St. Croix Health.
  • Monitor internal and external referrals and coordinate next steps with patients.
  • Make referral appointments, including preliminary appointments and testing.
  • Adjust care or service approaches to reflect the developmental levels and cultural differences of the population served.
  • Communicate with the care team in a timely manner and request or obtain patient reports as necessary.
  • Work with outside healthcare agencies to facilitate patient appointments and referrals.
  • Follow up with patients who request referral status updates.
  • Work with internal departments on care coordination.
  • Relay patient information to the care team in a timely manner.
  • Communicate with provider care teams to ensure patients' needs are met.
  • Follow up with patients to ensure they receive the care recommended by their provider.
