Requirements
  • A Michigan Licensed Social Worker license is required.
  • Two years of related work experience is required, preferably in a nursing home setting.
  • The ability to recognize the autonomy and dignity of all residents is required.
  • The ability to communicate care, concern, and dignity for residents is required.
  • The ability to place decision-making in residents' hands whenever possible and appropriate is required.
  • The ability to make prompt and accurate judgments regarding resident care and emergencies is required.
  • The ability to work and communicate effectively as part of a self-directed work team is required.
Responsibilities
  • Create a home environment that provides community members opportunities to enjoy the highest possible quality of life and provides psychosocial support to residents, families, and partners.
  • Maintain knowledge of each resident and their family, including a psychosocial profile and understanding of family dynamics.
  • Participate in care planning for resident needs.
  • Ensure that out-of-facility activities are available to residents.
  • Complete Minimum Data Set assessments accurately and on time.
  • Lead the Grievance Committee, hear resident concerns, and follow up on them promptly.
  • Complete investigations with assistance from the Executive Director.
  • Provide annual education on abuse, neglect, and misappropriation.
  • Issue Notice of Medicare Non-Coverage and Advance Beneficiary Notice information and support the appeal process.
  • Support and encourage social work interns as needed.
  • Include families in the planning process.
  • Understand the Medicaid Waiver process and support transitions of care.
  • Evaluate the effectiveness of programs and activities.
  • Provide weekly support to each level of care in Independent Living, Assisted Living, and Nursing.
  • Complete Preadmission Screening/Resident Review and Level of Care assessments on admission.
  • Document in accordance with organizational policies.
  • Complete admission assessments and resident discharges.
  • Perform case management.
  • Perform other duties as assigned.
Desired Qualifications
  • Post-secondary education is preferred.
  • Experience in a nursing home setting is preferred.
