Requirements
  • Registered Nurse currently licensed in the state in which the care is provided.
  • Previous experience in hospice, home health, geriatrics, or general medical and surgical nursing.
  • Demonstrated commitment to the hospice philosophy of care.
Responsibilities
  • Provide nursing assessment, planning, and care to maximize the comfort and health of patients and families.
  • Respond to all phone calls promptly and appropriately and make patient visits as necessary.
  • Complete after hours admissions and deaths as needed.
  • Initiate communication with attending physicians, hospice staff members, and other agencies to coordinate optimal care and use of resources for the patient and family.
  • Maintain regular communication with the attending physician and families concerning patient and family care.
  • Obtain data on physical, psychological, social, and spiritual factors that may influence patient and family health status and incorporate that data into the Plan of Care.
  • Maintain up to date patient records so that problems, plans, actions, and goals are accurately stated and changes are reflected as they occur.
  • Submit paperwork to the office as required by Medicare and Good Shepherd Hospice policies and procedures.
  • Instruct the patient and caregiver on medication administration, side effects, personal care, oral hygiene, lifting, moving, ambulation, exercise, disaster and safety management, and preparation for events preceding death.
  • Inform the Patient Care Manager of unusual or potentially problematic patient and family issues.
  • Attend hospice team meetings and other patient conferences, including care plan meetings.
  • Provide appropriate support at the time of death.
  • Share in providing 24-hour, seven days a week coverage to patients and families.
  • Evaluate hospice aides every two weeks and Licensed Practical Nurses at a minimum of once per month.
  • Develop an understanding of hospice philosophy and maintain proficiency in pain management through individualized studies, in-services, or seminars.
