Requirements
  • A high school diploma or equivalent GED is required.
  • Strong attention to detail is required for working with medical records and safeguarding confidential patient information.
  • Ability to use electronic health records, medical documentation, record management processes, and healthcare compliance standards is required.
  • Ability to perform communication, data entry, medical records management, medical records systems, teamwork, and independent work is required.
Responsibilities
  • Learn the fundamentals of health records and health information management.
  • Learn the terminal digit order filing system to locate historical paper and microfilmed medical records.
  • Develop a general understanding of Epic Chart Desk.
  • Develop an understanding of policies and procedures documented in the systems policy repository specific to the role.
  • Process health records, including paper medical record scanning, adoption cases, chart destruction and retention, unidentified patient assignment and merges, date of death processing, and medical record assignment.
  • Answer phone calls, satisfy requests, and relay messages associated with the role.
  • Develop an understanding of scanning methodologies, including HIM scans, the Fax Image Management System, and clinic scanning.
  • Develop familiarity with all areas of health information management and the role each team plays.
  • Learn what a duplicate medical record is and how to identify and report it.
  • Develop a general understanding of chart corrections to appropriately direct callers and assist as needed.
  • Maintain accurate, organized, secure, and accessible patient records to support quality patient care.
  • Comply with medical, administrative, ethical, legal, regulatory, and organizational requirements and policies.
