Requirements
  • A Master's or Doctoral degree in Speech Language Pathology is required.
  • The candidate must be licensed as a Speech Language Pathologist by the state where services are furnished.
  • Knowledge of medical terminology is required.
  • Reliable transportation and valid automobile insurance coverage are required.
  • Employment is contingent upon successfully completing a background check.
Responsibilities
  • Provide the full range of speech-language pathology services ordered by a physician.
  • Participate periodically with other home care personnel in patient care planning.
  • Take the patient's initial history and complete the initial evaluation and skilled procedures.
  • Consult with physicians regarding treatment changes and write reports regarding patient progress.
  • Complete and submit an initial evaluation, plan of treatment, and goals to the physician for approval.
  • Write a progress note for each visit.
  • Complete a reevaluation when the expected duration of treatment is reached.
  • Write a discharge summary when the patient is discharged.
  • Obtain physician recertification every 60 days when continued treatment is required.
Desired Qualifications
  • Two years of experience as a Speech Therapist is preferred.
  • Home Health experience is preferred.

Its goal is to keep seniors safe and healthy at home by coordinating comprehensive, specialty care in their living environments.
