Job Description Summary
The Transplant RN Coordinator is primarily focused on the solid organ transplant adult kidney patient population. The Transplant RN Coordinator is responsible for providing comprehensive care to transplant patients across the care continuum with an emphasis on patient assessment, care planning and collaboration with the multidisciplinary team. The Transplant RN Coordinator will work closely with the transplant team to assess patient needs, develop individualized care plans and ensure seamless collaboration across disciplines. Alongside the provider, the Transplant RN Coordinator will be required to commute among Transplant clinics in the Greenville and Anderson areas to provide comprehensive care to both transplant candidates and recipients.

Additional Details: Coordinate all clinical care within all clinics to maximize safety, patient and co-worker satisfaction and quality outcomes.
Key responsibilities include: patient evaluation and management, education and support for patients and their caregivers as they navigate the complexities of the transplant process, medication management, lab review, coordination of care and patient advocacy. Clinic related duties include patient triage, blood draws, and basic assessments. Specific duties and expectations may vary depending on the clinic's policies and patient needs.
Minimum education and requirements: Bachelor’s degree in nursing required and three years related nursing experience. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA).
