  • Receives all requests for enrollment of new patients
  • Performs interview to establish needs of patient
  • New Patient Packet
  • Release of Information (ROI) for previous medical records
  • Reviews Narcotic Drug Policy (as applicable)
  • Assigns appropriate provider / site for new patient
  • Responsible for processing and tracking ROI and new patient records/chart
  • Responsible for entry of new patient into Electronic Medical Record
  • Responsible for monitoring/answering calls to New Patient Hotline
  • Responsible for scheduling initial appointment for new patient
  • Reviews new patient tracking report
  • Other duties, work hours, and ACHS placement may be assigned
