  • A minimum of 3-5 years of experience in an acute care hospital setting, with a strong understanding of managed care.
  • Commitment to cost-effective management of resources while achieving positive clinical outcomes.
  • Excellent analytical and critical thinking skills to evaluate patients' discharge needs and develop plans accordingly.
  • Strong organizational and problem-solving skills, with the ability to prioritize work independently.
  • Strong communication and interpersonal skills to work with stakeholders across the care continuum.
  • Sound clinical judgment and an understanding of medical conditions and terminology when coordinating care.
  • Ability to function independently within the scope of the position and licensure, department policies, and established goals.
  • Ability to discuss sensitive clinical and financial issues with patients and families while providing guidance on placement and referrals.
  • Ability to identify legal or risk-management issues that create barriers to successful care transitions and develop plans to address them.
  • Understanding of federal, state, and local healthcare standards and regulations.
  • Proficiency with computer applications such as Microsoft Office.
  • A Bachelor's degree in Social Work is required.
  • A current Massachusetts license as a Licensed Clinical Social Worker; Licensed Independent Clinical Social Worker or eligibility for it is preferred.
  • Ability to communicate effectively with all age levels served within the scope of the position and assigned areas.
  • Ability to perform the essential functions of the position without posing a direct threat to the health and safety of oneself or others, with or without reasonable accommodation.
Responsibilities
  • Evaluate the mental, emotional, and physical status of patients.
  • Manage care coordination needs while developing and executing discharge plans.
  • Create discharge plans based on initial assessments, with ongoing reassessments and modifications as required.
  • Communicate with doctors, nurses, home health companies, and other healthcare personnel and agencies.
Desired Qualifications
  • Experience with Meditech and Cerner.
  • Certification in Case Management.
  • A Master's degree.
