Requirements
  • A high school diploma or GED is required.
  • At least 1 year of experience in a health care setting is required.
  • Strong computer skills are required.
  • Ability to multitask and maintain attention to detail.
  • Ability to use appropriate protocols and resources, critical thinking, and decision-making skills to navigate patients through service lines for referred services.
Responsibilities
  • Manage outgoing home care and hospice referrals from initial processing through start of care.
  • Ensure each referral is accurate, timely, and directed to an appropriate participating provider.
  • Process and track referrals.
  • Verify insurance participation.
  • Monitor timely starts of care.
  • Coordinate Plan of Care documentation and provider signatures.
  • Maintain accurate patient information within the electronic medical record.
  • Communicate proactively and follow up with providers, families, facilities, and care partners throughout the referral process.

  • Experience in a physician office, home care, or hospice setting.

Optimal Care provides Medicare-certified home health services for seniors, guided by a nurse practitioner-led care model. A nurse practitioner visits patients at home and develops individualized care plans that cover behavioral health, dementia, geriatric, diabetic, wound, and orthopedic care. This in-home, NP-driven approach offers a broad suite of geriatric-focused programs rather than only post-surgical or rehabilitation services. Its goal is to keep seniors safe and healthy at home by coordinating comprehensive, specialty care in their living environments.
