Requirements
  • If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
  • If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.
  • State of Wisconsin NP or PA licensure (minimum).
  • If Nurse Practitioner: Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND Active board certification as a Nurse Practitioner in area of clinical practice (Adult/Gerontology Acute Care or Adult/Gerontology Primary Care with clinical experience in the hospital or previous ICU experience as Nurse or Nurse Practitioner).
  • If Physician Assistant: Active PA license, including prescriptive authority, in state(s) of practice AND Active PA board certification.
  • All Nurse Practitioner and Physician Assistant applicants must have: Active DEA license or ability to apply for such license prior to or upon hire.
  • All Nurse Practitioner and Physician Assistant applicants must have: Active license or eligibility to obtain active licensure in state of practice. This may include licensing in Wisconsin and other state(s) of practice.
Responsibilities
  • Independently care for the hospitalized patients in the UW Home-based Hospital Care Program.
  • Work with patients with acute and chronic disease.
  • Perform assessment, diagnosis, treatment, management, prevention, care coordination including multidisciplinary care rounds, and management of acute and chronic conditions.
