Requirements
  • Active Michigan Speech-Language Pathologist license in good standing.
  • Strong clinical reasoning and the ability to operate independently in the field.
  • Ability to identify red flags and escalate changes in condition to a registered nurse or physician.
  • Strong caregiver education skills, including teach-back and carryover planning.
  • Strong communication and documentation skills.
  • Reliable follow-through on orders, frequencies, and patient goals.
  • Current driver's license and comfort with daily travel across the assigned territory.
Responsibilities
  • Own the patient's Speech-Language Pathologist plan, progression, and functional outcomes.
  • Complete comprehensive in-home Speech-Language Pathologist evaluations and deliver skilled Speech-Language Pathologist visits.
  • Assess and treat aphasia and other communication disorders as ordered or clinically indicated.
  • Assess and treat cognitive-communication impairment involving attention, memory, and executive function that affects safety and independence.
  • Perform dysphagia screening and assessment and provide swallow-safety education within home-health scope and orders.
  • Build and execute a clear plan of care with measurable, functional goals.
  • Coordinate with the multidisciplinary team by communicating changes in condition, aligning on orders, and keeping the care team moving.
  • Coach patients and caregivers with practical strategies and routines that support carryover.
  • Document promptly and thoroughly in a defensible, efficient, and consistent manner.
Desired Qualifications
  • Prior home-health experience.
  • Neuro experience involving stroke, aphasia, or cognitive impairment.
  • Dysphagia-related experience appropriate for home health.
