- Current American Heart Association Basic Life Support certification, including CPR, First Aid, and AED, is required.

- The employee must use conversational speech for effective verbal communication and hear equipment alarms, overhead pages, and direct verbal communication in areas with potentially high noise levels.
- Facilitate the hospital discharge process and patient assessment and engagement in the program.
- Perform medical assessment intake and discharge planning of care.
- Maintain current guest charts daily using the existing customer relationship management database.
- Conduct medical and wellness checks, provide medication regimen education and preventive measures, and follow up on medical insurance.
- Coordinate and provide linkages for guests' medical appointments and follow-up care.
- Collaborate with Intake to confirm eligibility for recuperative services and coordinate required durable medical equipment and home health needs.
- Participate in weekly internal and external case conferences, board review team meetings, monthly staff meetings, trainings, and in-services.
- Advocate for guests when needed and in accordance with LVN manager instructions.
- Manage a caseload of at least 15 guests, with flexibility to manage more based on facility needs.
- Track and complete guest information in the existing database, including progress and plans of care.
- Complete required documentation during administrative hours.
- Perform administrative tasks such as charting, preparing guest summaries, and answering phones.
- Follow organizational policies and procedures and maintain a positive professional image and organizational dress code.
- Accommodate schedule changes to meet facility needs.
- Complete additional duties as required without impeding primary responsibilities.
- Recent work experience with homeless patients is preferred.
- Bilingual Spanish ability is preferred.
