Requirements
  • Graduation from an approved certification, program, accredited school, undergraduate or graduate school, or postgraduate education sufficient to demonstrate the ability to perform the job.
  • A valid and unencumbered license or verification to practice as an LPN/LVN in the state where services are provided.
  • Professional experience required by state regulations to perform the job.
  • Successful completion of assigned screening and competency testing with a proficiency score of at least 80%.
  • Reliable transportation.
  • A current tuberculosis test result or chest X-ray.
  • Current cardiopulmonary resuscitation certification in accordance with agency policy.
  • Flexibility, organization, and the ability to exercise independent and sound judgment.
  • Ability to perform the required physical activities, including sitting, standing, stopping, bending, kneeling, walking, balancing, crouching, crawling, pushing, pulling, and reaching.
  • Vision and hearing within normal limits, with or without aid.
  • Ability to frequently lift or move up to 15 pounds and occasionally lift or move 100 pounds or more, when applicable.
  • Ability to perform the anticipated level of physical assistance required for safe client care.
Responsibilities
  • Provide skilled nursing as ordered in the plan of treatment and nursing care plan.
  • Provide skilled nursing care during each shift or visit in accordance with physician orders.
  • Identify and follow through on new problems, changes, and resolutions in patient condition.
  • Notify the director or supervisor of new problems, changes, or resolutions.
  • Notify the physician to obtain necessary intervention orders in accordance with state regulations.
  • Document clinical changes and interventions in clinical notes, supplemental physician orders, and the nursing care plan.
  • Use aseptic technique when required.
  • Perform treatments and administer medications according to the care plan.
  • Function as a team member under the direction of a registered nurse.
  • Work shifts according to the plan of treatment.
  • Communicate skilled observations and significant changes in patient status to the Director of Clinical Services or Clinical Manager.
  • Request modified or additional orders based on patient clinical status and nursing judgment.
  • Document physician collaboration in the medical record.
  • Use and document appropriate utilization of available services.
  • Participate in agency-mandated activities, including in-service training, as requested by the agency director.
  • Follow agency policies, procedures, and handbook requirements.
  • Complete and submit clinical documentation accurately and promptly according to agency policies and guidelines.
  • Report on time and as scheduled to complete work within the designated time.
  • Provide care in private homes and travel frequently to complete home health visits.
