Requirements
  • More than three years of experience in healthcare operations management, preferably in home healthcare or hospice services.
  • Demonstrated practice management experience, including clinical workflow oversight, billing, and utilization management.
  • Experience managing on-call schedules and ensuring 24/7 clinical coverage.
  • Strong knowledge of healthcare regulations, compliance, and quality assurance standards.
  • Proven ability to manage budgets, financial performance, and operational efficiency.
  • Strong problem-solving skills and the ability to make decisions in a fast-paced environment.
  • Commitment to delivering high-quality patient care and exceptional customer service.

Responsibilities
  • Oversee daily operations of home-based primary care services, including scheduling patient visits and new patient intake.
  • Build and maintain relationships with key stakeholders, referral sources, and community partners.
  • Ensure compliance with federal, state, and local regulations.
  • Develop and implement operational policies and procedures to improve efficiency and quality of care.
  • Monitor and manage budgets, financial performance, and resource allocation.
  • Conduct regular audits and quality assurance checks to maintain high standards of care.
  • Oversee the on-call schedule, coordinating rotations across clinical and medical staff to ensure 24/7 coverage, continuity of care, and clear escalation protocols.
  • Collaborate with credentialing and licensure functions to maintain collaborative practice agreements and quality oversight.
  • Oversee the residency program in partnership with academic affiliates, managing rotation schedules, preceptor assignments, accreditation compliance, and resident onboarding.
  • Recruit, train, and supervise a team of healthcare professionals in partnership with Talent Acquisition.
  • Develop and implement staff schedules to ensure adequate coverage and optimal patient care.
  • Conduct performance evaluations and provide ongoing training and development opportunities.
  • Foster a positive and collaborative work environment that promotes teamwork and professional growth.
  • Ensure patient care plans are developed and executed in accordance with best practices and patient needs.
  • Work collaboratively with the Care Coordination function to ensure clinical needs are met.
  • Address patient and family concerns promptly and effectively.
  • Collaborate with healthcare providers, physicians, and other stakeholders to coordinate care and services.
  • Monitor patient outcomes and satisfaction, making necessary adjustments to care plans and services.
  • Maintain accurate and up-to-date records and documentation.
  • Implement and monitor quality assurance programs to enhance service delivery.
  • Stay informed of industry trends, best practices, and regulatory changes.
  • Support the development and implementation of strategic plans to grow and expand home healthcare and hospice services.
  • Identify opportunities for improvement and innovation in service delivery.
  • Collaborate with senior management to set organizational goals and objectives.
