Requirements
  • A Bachelor's Degree in Business or Public Administration, Behavioral or Social Sciences, or a degree related to the agency program that demonstrates the capacity for the knowledge and skills; and two years experience coordinating or administering a program
  • Any combination of experience or education equivalent to five years of experience that typically supports the knowledge and skill requirements listed for the classification.
Responsibilities
  • Coordinating all IDT meetings
  • Leading the development of Treatment Care Plans (TCPs)
  • Ensuring the integration of all aspects of patient care into the TCP
  • Incorporating Behavior Support Plans, Emergency Treatment Care Plans, STARTs, and other key assessments to create a comprehensive and individualized approach to care.

  • Collaborative Leadership – Ability to guide interdisciplinary teams, coordinate input from multiple disciplines, and foster strong team functioning.
  • Clinical Knowledge & Analytical Skills – Skilled in integrating assessments, behavior support plans, and risk data into comprehensive treatment care plans.
  • Communication & Facilitation – Strong written and verbal communication skills to lead IDT meetings, keep teams on track, and effectively engage patients, families, and staff.
  • Attention to Detail & Organization – Ability to manage complex scheduling, documentation, compliance with hospital policies, and ensure accuracy in treatment care planning.
  • Cultural Competence & Patient-Centered Approach – Demonstrates respect for diversity and commitment to developing individualized, recovery-focused, and culturally responsive care plans.
