Requirements
  • A high school diploma or GED is required.
  • One to three years of related work experience is required, or two years of related work experience without a related Bachelor's degree.

  • The role requires light physical work, including frequently exerting up to 20 pounds of force and standing or moving throughout the hospital campus.
Responsibilities
  • Determine, independently or with clinicians, the resources that optimally meet patients' and families' needs and independently recommend services.
  • Maintain a caseload with multiple patient contacts per day, including phone calls, paperwork, and face-to-face contacts.
  • Research best practices and programs to meet patients' needs and function as the primary healthcare contact for psychosocially complex patients when needed.
  • Document patient care and services promptly and appropriately in medical records using EPIC electronic medical records.
  • Advocate on patients' behalf through written and verbal communication.
  • Assist patients and families with accessing entitlement programs.
  • Provide appropriate medical information to extended care facilities, the Department of Transitional Assistance, and other organizations as requested.
  • Conduct staff and community-based training on available resources.
  • Provide staff with updates on procedural requirements and changes from state and local agencies.
  • Document and advise the supervisor and clinical staff about obstacles to obtaining services that require social action.
  • Develop liaisons with community and governmental agencies to facilitate negotiation of complex systems.
  • Complete MassHealth applications for the needed patient population.
  • Coordinate community initiatives such as the car seat program and coat drives.
  • Participate in creating and managing schedules, including on-call schedules.
  • Oversee onboarding and offboarding processes for the department.
  • Assist with other department tasks as assigned.
Desired Qualifications
  • A Bachelor's degree in Social Work or Human Services is preferred.
  • A Licensed Social Worker license is preferred.

Beth Israel Lahey Health rolls out Heidi's AI scribing to all physicians after 90% report better patient presence

Beth Israel Lahey Health is rolling out Heidi's ambient AI scribing tool to all its physicians following a successful trial across its facilities. The health system, which serves over 6,000 providers across 14 hospitals and 175 primary care practices, prioritises addressing physician burnout, with nearly half of US physicians reporting burnout. During a six-month trial offering Heidi to 1,000 providers, the majority adopted the technology. Survey results showed 89% were satisfied with note quality, 90% felt more present with patients, 74% reduced after-hours work time, and 82% experienced reduced cognitive load during visits. The implementation demonstrates how ambient AI can deliver immediate relief to physicians whilst benefiting patients and the healthcare system. Heidi supports over 2 million consultations weekly globally and has raised $96.6 million from investors including Point72 Private Investments and Blackbird.
