Requirements
  • Demonstrated customer service skills and the ability to de-escalate situations.
  • Computer and keyboarding skills, including Microsoft Office, Excel, PowerPoint, and Adobe.
  • An interest in serving elderly people or people with disabilities.
  • Excellent verbal and written communication skills.
  • Ability to provide customer service to people of different ages, races, genders, socioeconomic statuses, and special needs.
  • Ability to manage multiple projects and deadlines in a fast-paced environment.
  • Ability to drive to work-site locations.
  • A valid driver's license and appropriate auto insurance for business travel.
  • Ability to lift 25–50 pounds and bend, twist, kneel, reach, push, pull, and carry.
Responsibilities
  • Assist with annual care planning in collaboration with Intake Coordinators and ensure services meet members' needs and agency and state requirements.
  • Build and maintain client data and collaborate with the Vibrant Administration team to support appropriate service delivery.
  • Coordinate annual care meetings as needed.
  • Travel to members' homes and designated locations to obtain signatures on care plans, service agreements, consent forms, and other required documentation.
  • Maintain accurate records of completed visits and collected documentation.
  • Ensure signed documents are submitted and filed according to agency policies and state requirements.
  • Answer phones, emails, and text messages to provide customer service to field staff, clients, families, and referral sources.
  • Assist with documentation requirements, intake, annual client paperwork, and onboarding materials.
  • Perform office functions such as scanning, sorting and preparing mail, filing, and preparing documents.
  • Run operational reports as directed by management or administration.
  • Support managers and employees with daily office operations and general administrative activities.
  • Handle confidential and time-sensitive information with discretion and accuracy.
  • Represent the agency professionally and provide professional customer service to members in their homes and communities.
