Requirements
  • Must have a valid Speech Language Pathologist license in the state or states of practice, or proof of license eligibility.
  • Must have a valid driver's license or reliable, independent transportation to and from patients' residences.
Responsibilities
  • Provide outpatient speech-language rehabilitation services in patients' homes, assisted living facilities, and independent living facilities.
  • Organize and provide speech-language services to facilitate rehabilitation.
  • Direct patient participation in selected tasks to restore, reinforce, and enhance performance.
  • Facilitate learning of skills and functions essential for communication and swallowing to diminish or remediate disorders or deficits.
  • Direct activities that promote and maintain health within the scope of practice and professional Code of Ethics.
  • Supervise clinical fellows, temporary licensees, students, and support personnel as necessary in accordance with state licensure requirements and professional standards.
  • Adhere to department and patient treatment schedules, clinical and billing documentation requirements, and work-related meetings.
  • Attend and contribute to patient care, staffing conferences, and related meetings.
  • Make referrals to other disciplines and agencies to address patients' comprehensive needs.
  • Consult with rehabilitation team members and physicians to select appropriate therapeutic regimens based on each patient's needs and capabilities.
  • Adapt programs and plans of care to individual patient needs.
  • Promote a safe environment and eliminate fire and safety hazards.
  • Order supplies and equipment as necessary.
  • Clean and maintain equipment and identify and report repair needs.
  • Provide patient- and family-centered service that recognizes individual needs and rights.
  • Screen, examine, and evaluate patients; establish diagnoses; identify impairments; determine predicted improvement and treatment timelines; identify precautions and contraindications; and design plans of care.
  • Develop treatment goals and methods with patients and caregivers, implement speech-language pathology treatment plans, and complete related documentation and record keeping.
  • Apply therapeutic techniques for cognitive communication, speech intelligibility, oral motor skills, swallowing disorders and dysphagia, perceptual abilities, orientation, memory, pragmatics, psychosocial expression, functional communication, and case management in home and community settings.
  • Coordinate acquisition of adaptive communication devices and instruct patients in their use.
  • Facilitate identification of hearing deficits and refer patients for assessment, diagnosis, rehabilitation, or personal amplification.
  • Select activities suited to each individual's physical and mental capacity, intelligence level, and interests.
  • Instruct patients in skills and techniques for therapeutic activities and evaluate their progress, attitudes, behavior, and rehabilitative potential.
  • Perform routine reexaminations to modify or progress treatment plans.
  • Coordinate home and community care with interdisciplinary team members and the referring physician.
  • Perform other related duties as required.
