Requirements
  • A high school diploma or equivalent is required.
  • Two years of experience in medical records in a long-term care, hospital, or related healthcare setting is required.
  • Working knowledge of diagnosis codes is required.
  • The ability to read, write, speak, and understand English is required.
  • The employee must be able to sit for up to 8 hours per day, stand for up to 8 hours per day, and walk for up to 6 hours per day.
  • The employee must be able to lift, transfer, push, or pull equipment and supplies weighing up to 25 pounds.
  • The employee must work weekends, evenings, and holidays as scheduled.
  • The role involves occasional exposure to residents or patients with various disease processes and has a Risk Category III exposure to bloodborne diseases.
Responsibilities
  • Review all records on the nursing units.
  • Assist in developing procedures to ensure records are properly assembled, coded, signed, indexed, and filed.
  • Provide clerical support for the nursing management team.
  • Assist in planning, developing, organizing, implementing, evaluating, and directing medical records in accordance with established policies and procedures.
  • Collect, scan, and attach resident and patient information to the electronic medical record.
  • Return incomplete electronic medical records to the Nursing Department for completion.
  • Gather and organize information for insurance appeals.
  • Assign International Classification of Diseases, 10th Revision diagnosis codes as assigned.
  • Maintain schedules and gather and organize information for insurance recertifications.
  • Initiate, prepare, and ensure completion of Medicare certifications and recertifications as assigned.
  • Notify residents, patients, and families of non-coverage for Medicare or insurance recipients as assigned.
  • Complete five-day admission audits as assigned.
  • Audit electronic medical records for completeness and report findings to the Director of Nursing, Administrator, or Unit Manager.
  • Gather documentation needed for federal, state, and city surveys.
  • Attend all-employee and departmental meetings and scheduled or directed in-service training.
  • Assist in establishing procedures for collecting electronic medical records.
  • Advise the Director of Nursing on federal and state laws concerning medical records.
  • Ensure all staff follow established electronic medical record policies and procedures.
  • Review department policies and procedures at least annually and participate in recommended changes.
  • Collect and audit physician signatures and notify the Director of Nursing or Unit Manager when signatures cannot be obtained.
  • Maintain the confidentiality of resident and patient information.
