Requirements
  • A bachelor's degree in a related human services field is required.
  • A valid driver's license and vehicle are required to travel.
  • Knowledge of social services and medical needs of psychiatrically disabled persons is required.
  • Knowledge of available resources to assist psychiatrically disabled persons is required.
  • Knowledge of appropriate record-keeping procedures is required.
  • Sensitivity to multicultural differences is required.

Responsibilities
  • Provide direct services to individuals as specified in their treatment plan.
  • Work closely with the State Operated Mental Health Facility.
  • Work closely with the community support treatment team triage.
  • Develop plans to refer, link, and place individuals with community-based treatment and other needed resources.
  • Refer, link, and place individuals with recreation, transportation, financial and medical entitlement, and vocational rehabilitation services, as appropriate and needed.
  • Coordinate inter-agency and intra-agency services.
  • Conduct outreach and case finding.
  • Determine eligibility and conduct comprehensive mental health assessments of service needs in accordance with Department of Human Services time guidelines.
  • Develop the comprehensive master individual Treatment Plan with input from the client or family unit, in accordance with Department of Human Services time guidelines.
  • Regularly review each client's Treatment Plan to assess effectiveness and the need for specific plan elements, with input from the client or family unit.
  • Refer, link, and place individual clients with needed medical health services within the community.
  • Monitor and evaluate all services provided to individual clients.
  • Locate appropriate housing facilities as needed.
  • Provide supportive counseling to individual clients or family units as needed.
  • Provide problem-solving assistance to individual clients or family units as needed.
  • Provide public education on available services and the needs of mentally ill people as opportunities arise or as assigned by the Director of Mental Health Case Management.
  • Document all services provided in daily progress notes.
  • Perform Office of Health Services billings daily.
  • Perform other duties as assigned by the Director of Mental Health Case Management.
Desired Qualifications
  • Knowledge of languages other than English, particularly Spanish, is helpful.
