Requirements
  • Doctorate degree pharmacy (Pharm.D.) from an ACPE-accredited college of pharmacy.
  • A one (1) year accredited Post Graduate Year 1 pharmacy residency.
  • A one (1) year PGY2 Pharmacy Residency in Oncology/Hematology OR BCOP certification with minimum of 2 years of oncology experience that includes pharmacy regulatory compliance, compiling medication use evaluation (MUE) data or equivalent quality assurance (QA) data for Pharmacy and Therapeutics (P&T) Committee.
  • Valid California state pharmacist license with no disciplinary action against the licensee that prohibits the licensee from practicing pharmacy.
  • Valid CA driver’s license
  • Valid NPI (National Provider Identifier) number issued by the Center for Medicare and Medicaid Services (CMS)
  • This position is responsible for the assessment, treatment, and care of adolescent, adult, and geriatric patients.
Responsibilities
  • Manage the oncology formulary by evaluating and onboarding new agents.
  • Determine the appropriate site of care for patients receiving oncology treatment, considering factors such as inpatient, outpatient, or regional campuses.
  • Oversee the review process for off-label, non-formulary, or other non-standard use of antineoplastic agents.
  • Promote appropriate, standardized and guideline-based antineoplastic utilization across all El Camino Health sites where appropriate.
  • Review provider requests for exception orders and evaluate for literature support to guide the prior authorization process when applicable.
  • Collaborate with relevant stakeholders, including physicians and other healthcare professionals to ensure the safe and effective use of these agents.
  • Work closely with pharmacy informatics teams to contribute to the development and revision of Electronic Health Records resources.
  • Ensure that EHR resources support the safe and effective use of oncology medications and adhere to current guidelines and best practices.
  • Coordinate, facilitate, and provide education, training, competency assessment and performance improvement to pharmacists, pharmacy residents and other healthcare professionals for the appropriate use of antineoplastics as well as on best practices and the latest developments in oncology pharmacy.
  • Collaborate with other disciplines to develop and implement best practices across the institute related to antineoplastic utilization.
  • Contribute to the institution’s antineoplastic stewardship program through compiling data and metrics on antineoplastic utilization in the outpatient and inpatient settings, reporting data and findings to relevant stakeholders.
  • Monitor antineoplastic utilization practices to identify areas for improvement and implement necessary changes.
  • Build and maintain databases and dashboards that support the work of the Antineoplastic Stewardship Program as required.
  • Collaborate with the Department of Quality and Patient Safety in the assessment, mitigation and dissemination of antineoplastic-related safety initiatives.
  • Stay updated on the latest advancements in oncology pharmacy and ensure that the pharmacy services align with current standards of care.
  • Contribute to the development and maintenance of policies and procedures to support the safe use of antineoplastic medications across all ECH sites and promote and enhance patient outcomes through safe, effective, cost-effective, and appropriate medication therapy use for patients receiving antineoplastic therapy.
