- Collaborate with department leadership and staff, donors and families, physicians, specialists, and ancillary staff in planning, organizing, coordinating, and executing specified program activities.

- Assist with coordinating safe, high-quality patient care and integrate services and prescribed treatment among hospital programs, service sites, departments, and physicians.
- Coordinate and schedule medically related appointments across the healthcare continuum with members or their designees.
- Facilitate members’ participation in care decisions and advocate appropriately on their behalf.
- Adhere to health-system scripting and seek assistance when clarification is needed.
- Recognize appropriate opportunities to capture and relay vital information to program leadership to support philanthropic activities and member retention.
- Ensure physician orders needed to initiate treatments and scheduling are available and viable, and intervene with physicians and the program team to rectify issues.
- Intervene appropriately to reduce anxiety and improve patients’ ability to follow through with appointments and prescribed care.
- Involve Registered Nurses and leadership to provide current medical information about diagnostic testing and planned treatments, and refer members to an appropriate entity when needs exceed the Licensed Vocational Nurse’s scope or knowledge.
- Escort patients to and from departments for tests and treatments, assess the need for adaptive equipment and wheelchairs, remain with patients as requested, and take appropriate customer-service actions to ensure a high-quality patient experience.
- Recognize potentially critical situations and take action to minimize adverse outcomes.
- Promote positive perceptions of the program and communicate appreciation for donors’ role in the organization’s fiscal health.
- Share knowledge and serve as a professional resource to colleagues and other healthcare team members.
- Train, orient, coach, and support new hires to promote safe care, quality, and customer service.
- Use evidence-based practice and demonstrate commitment to continuous improvement.
- Support department morale and promote a positive department image when interacting with health-system staff and professionals.
- Address service issues directly or inform and engage program leadership, and involve line managers in service recovery when necessary.
- Provide patients with information about how to contact program staff.
- Treat patients, staff, physicians, volunteers, and visitors with kindness, empathy, dignity, and respect.
- Use critical thinking and collaborate with leadership, peers, and staff to improve and streamline workflows while conserving health-system resources.
- Assist other team members when necessary.
- Perform other duties as assigned.
