Requirements
  • Three years of experience in a clinical setting.
  • An active Tennessee registered nurse license, or a license in the applicant's state of residence if that state participates in the Nurse Licensure Compact Act, or a Master's in Social Work with an active unrestricted license such as LCSW, LMSW, or LAPSW.
  • Exceptional customer service skills.
  • Residence in Rhea County, Tennessee.
  • Availability for an 8:00 a.m. to 5:00 p.m. Eastern Time schedule with no on-call requirement.
  • A valid driver's license.
  • A TB skin test for coordinators who work in the field.
  • Basic Microsoft Office and email skills.
  • Effective time-management skills.
  • Strong oral and written communication skills.
  • Strong interpersonal and organizational skills.
  • Compliance with applicable auto-insurance requirements and maintenance of an acceptable motor vehicle record when routinely driving for company business.
  • Completion of a digital literacy assessment.
  • Various immunizations and associated medical tests may be required.
Responsibilities
  • Partner with members and families to identify needed supports and direct services that support personal goals related to health, employment, and independent or community living.
  • Collaborate with clinical and social-support colleagues to meet each member's physical, behavioral-health, and long-term service needs.
  • Conduct thorough, objective face-to-face visits and assess each member's situation, including physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs and expectations.
  • Use authorization criteria to approve appropriate home- and community-based services, confirm that services are being provided, and ensure members' needs are met.
  • Travel to members' homes for visits and manage demands and requests from internal and external stakeholders.
