- Plan, coordinate, and implement spiritual support activities
- Perform a spiritual assessment within five days of admission to hospice
- Attend interdisciplinary team meetings and collaborate with the team
- Coordinate with or refer to local clergy as requested by the patient and family
- Document clearly, accurately and timely in the patient record
- Follow agency policy & procedures and adhere to professional boundaries
- Comply with HIPPA and confidentiality agreement
- Assist families in planning funeral and memorial services
- Collaborate with the bereavement coordinator to plan hospice memorial services
- Attend interdisciplinary team meetings and report current issues
- Coordinate care with other professionals to meet the emotional and psychological needs of the patient and family
- Conduct a risk assessment of complicating bereavement factors
