Responsibilities
  • Plan, coordinate, and implement spiritual support activities
  • Perform a spiritual assessment within five days of admission to hospice
  • Attend interdisciplinary team meetings and collaborate with the team
  • Coordinate with or refer to local clergy as requested by the patient and family
  • Document clearly, accurately and timely in the patient record
  • Follow agency policy & procedures and adhere to professional boundaries
  • Comply with HIPPA and confidentiality agreement
  • Assist families in planning funeral and memorial services
  • Collaborate with the bereavement coordinator to plan hospice memorial services
  • Attend interdisciplinary team meetings and report current issues
  • Coordinate care with other professionals to meet the emotional and psychological needs of the patient and family
  • Conduct a risk assessment of complicating bereavement factors
