Requirements
  • Minimum of one year of experience as an occupational therapist in an acute setting is required.
  • An active occupational therapist license in the state of employment is required.
  • Home care experience is desired.
  • Ability to effectively communicate with patients, families, physicians, and co-workers is required.
  • Knowledge of therapy best practices is required.
  • Ability to make appropriate occupational therapy judgments is required.
  • Ability to identify situations and handle them with an appropriate solution is required.
  • Ability to be detail-oriented and observant is required.
  • Ability to prioritize work and complete it in a timely manner is required.
  • A valid driver's license and auto insurance listing the employee as a driver are required.
  • Reading, speaking, and writing English are required.
  • Ability to use computers or personal digital assistants is required.
  • Ability to walk, bend, stand, sit, drive, lift up to 50 pounds with or without assistance, stretch, reach, climb, and perform hand and finger movements is required.
  • Ability to distinguish smell and temperature and to hear and see for close work is required.
Responsibilities
  • Provide skilled occupational therapy in patients' homes as ordered by the attending physician.
  • Evaluate patients' level of functioning using diagnostic and prognostic procedures.
  • Develop individualized occupational therapy plans of care and adjust them as patients' needs change.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and appropriate agency staff.
  • Front-load visits for high-risk patients to help decrease rehospitalizations.
  • Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive or active exercise and activity.
  • Guide patients in therapeutic, creative, and self-care activities to improve functioning.
  • Arrange outpatient services that cannot be provided in the patient's home.
  • Promote continuity of care through appropriate admissions, transfers, and discharges.
  • Counsel patients and families about diseases or injuries and their management throughout occupational therapy.
  • Involve patients and families in developing plans of care.
  • Instruct patients and family members in the proper use of wheelchairs, braces, walkers, crutches, canes, and other prosthetic or orthotic devices.
  • Coordinate with the durable medical equipment department and medical social worker to ensure patients and families understand their financial responsibility for needed equipment.
  • Teach patients preventive measures and support sound decisions to help decrease rehospitalizations.
  • Listen to patients and family members.
  • Oversee and supervise the total care provided by certified occupational therapy assistants.
  • Evaluate plans of care based on patient responses to treatment and medical conditions, and adjust them as needed.
  • Communicate results of supervisory visits to certified occupational therapy assistants.
  • Complete clinical documentation in accordance with agency protocols and Medicare and federal guidelines.
  • Document treatment, assessments, and patient education.
  • Complete chart audits at the agency site as requested.
  • Communicate schedule changes to the scheduler.
  • Participate in on-call requirements and case conferences.
  • Attend in-service training and mandatory agency meetings.
  • Participate in the orientation and training of new employees.
  • Maintain a professional appearance as a company representative.
  • Follow agency policies and procedures.
  • Drive to patients' primary locations for scheduled visits and attend assigned visit locations daily.
  • Document subsequent, discharge, evaluation/recertification, and resumption-of-care visits within 24 hours.
  • Submit all required documentation within specified company requirements.
  • Follow the plan of care within the scope of practice for a licensed occupational therapist and applicable state requirements.
  • Perform other assigned duties.
