- A Master of Social Work degree.
- An active state license to practice as a Licensed Clinical Social Worker.
- At least two years of full-time experience in care coordination, discharge planning, or case management in a medical setting within the last four years.
- Basic Life Support certification.
- Use clinical assessment skills and maintain procedures to identify sponsors whose family members have special medical and educational needs in a timely manner.
- Use a multidisciplinary and collaborative approach with installation youth and childcare facilities and officer and enlisted spouses groups to ensure effective outreach and identification of special needs.
- Ensure family members of Air Force and Space Force personnel are appropriately Q coded and enrolled in the Exceptional Family Member Program.
- Ensure active-duty sponsors known to the Air Force Personnel Center and local Military Personnel Section and/or Commander Support Staff, where applicable, who have family members with special needs are identified in Air Force medical special-needs data management systems.
- Provide oversight for base-level data entry in Air Force-provided data management systems used to manage the Exceptional Family Member Program—Medical.
- Provide training to installation personnel and medical staff on the Exceptional Family Member Program and provide consultation as needed to support base-wide implementation of the medical component.
- Ensure all Family Member Travel Screening requirements are implemented by Exceptional Family Member Program—Medical staff.
- Coordinate enrollment and travel screening for active-duty family members and, upon request, travel screening for families of Department of Defense civilian-sponsored assignments going to an overseas base, including sister-service Family Member Travel Screening processing.
