Join our team and make a positive impact on the lives of others! We are looking for an Intake Patient Navigator for our new team in Colorado.
This is a hybrid position with a required commute of 2-3 days into the FreedomCare hub in Denver, CO.
Department & Position Overview:The Intake Patient Navigator role is responsible for connecting with inbound callers to promote the uniqueness of FreedomCare and our programs in Colorado. This role combines the initial lead and intake process for both patients and caregivers.
Your day to day will involve reaching out to inbound leads and providing information to all potential caregivers and patients. You will be collecting and validating information to begin the intake process for interested patients, and reengaging those who may need a little more nurturing. The goal of this role is to educate, provide awareness of our In-Home Support Services (IHSS) and grow the number of patients and caregivers that join FreedomCare.
Everyday You Will:
- Manage large amounts of inbound and outbound calls in a timely manner
- Turn interested callers into FreedomCare Patients and Caregivers
- Engage potential patients and caregivers who are looking for more information regarding IHSS program and FreedomCare in general
- Explain the enrollment process in detail to all potential members in a way that hopeful and exciting
- Explaining the benefits of enrolling in FreedomCare and why we are a unique team
- Stay updated on Medicaid and health plan policies to ensure ease of enrollment
- Nurture interested individuals into signing up with FreedomCare
- Collect and validate current information needed to sign up
- Follow up with our patients/caregivers and healthplans via follow-up calls, emails and texts
- Ensure a seamless, complete and effective enrollment process for all prospective patients and caregivers into the IHSS program
- Be the initial point-of-contact for calls from prospective clients, caregivers or pending enrollees
- Manage a high open caseload until the enrollment process is complete
- Consistently meet monthly completed start of care
- Provide exceptional customer service and positive experience for potential patients/caregiver
- Navigate multiple systems and portals to determine eligibility and obtain authorization
- Regularly achieve monthly new enrollee goals
Ideal Candidate Will Possess:
- Candidate based in Denver, CO and able to travel on occasion to our FreedomCare Denver office
- Experience in high-volume inbound and outbound call environment
- Motivated to help patients/caregivers with the drive to exceed open case targets and metrics
- Must have poise and patience when dealing with clients
- Excellent written and verbal skills; ability to comfortably resolve issues over the telephone
- Computer proficiency required; strong typing skills and ability to leverage technology to identify, research and solve customer issues
- Keen attention detail, metrics driven, excellent time management skills
- High empathy and collaborative team player
Nice-to-Haves:
- Two years of relevant experience in a similar position is preferred
- Healthcare/startup experience
- Bilingual in Spanish, or other languages desired

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $16.00 and $20.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
