Requirements
  • Valid and active Ohio Licensed Social Worker license issued by the CSWMFT Board.
  • Bachelor’s degree in Social Work or Master’s degree in Social Work from a Council on Social Work Education-accredited program.
  • At least 2 years of clinical experience, including 1 year of experience with elders or adults with disabilities.
  • A valid driver’s license and an insured vehicle for travel within Ohio.
  • Demonstrated ability to evaluate operational tasks and set, achieve, and measure targeted outcomes in a hybrid and field-based environment.
  • Experience in case management and the ability to work autonomously in a fast-paced, ambiguous, and accountable environment.
  • Ability to communicate effectively in writing and verbally across diverse internal and external audiences.
  • Experience using a customer relationship management system.
Responsibilities
  • Interpret the agency’s philosophy and objectives to clients, families, and other stakeholders.
  • Ensure compliance with HIPAA, Ohio Department of Medicaid and Ohio Department of Aging rules and regulations, provider policies, FreedomCare procedures, and applicable state and federal mandates.
  • Conduct initial in-person home assessments with patients and caregivers to verify compliance needs and ensure the plan of care is established and clearly communicated.
  • Monitor patients and caregivers day to day, including monthly contact and a combination of telephonic and in-person visits with no more than 60 calendar days between in-person visits.
  • Monitor patient enrollment, credentialing, claims operations, prior authorization appeals, patient satisfaction, caregiver support, program integrity, and related reporting, and triage issues to the appropriate internal staff.
  • Ensure service quality meets the required plan of care by providing individualized coaching, continuing education, and reviews of caregiver goals and needs.
  • Maintain detailed and accurate case records, including written records, reports, and notes documenting significant changes and observations after client visits or contacts.
  • Act as the escalation point and provide input on the client’s plan of care, including the physician’s plan of treatment and problem list, while implementing and monitoring changes as needed.
  • Perform other job-related duties as directed by the supervisor.
