Requirements
  • A high school diploma or GED is required.
  • Two or more years of experience in social service delivery with elderly people, people with disabilities, and/or family populations are required.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project is required.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for participants served is required.
  • Good communication, comprehension, and interpersonal skills are required.
  • Working knowledge of Windows, Microsoft Office Suite, and the World Wide Web is required, demonstrated by a passing score on a computer literacy test.
  • Ability to speak, read, write, and understand English is required.
  • Ability to apply common-sense understanding to carry out detailed instructions is required.
  • Ability to perform basic arithmetic operations, including operations involving money, with a calculator or Excel spreadsheet is required.
  • Occasional travel or travel as directed by contract and department requests is required.
  • Completion of annual education requirements, including Compliance, Privacy, Security, and HIPAA training, is required.
  • Compliance with applicable organizational policies, facility handbooks, and federal, state, and local laws is required.
Responsibilities
  • Coordinate programs and activities for eligible individuals on a group basis.
  • Serve as a liaison to community agencies, network with community service providers, and identify new services available to eligible individuals.
  • Monitor community-agency services and document participant use in the designated documentation management system.
  • Manage supportive services where appropriate.
  • Develop and maintain a resource directory of state and local service providers.
  • Organize educational events covering health care, agency support, life skills, and referral sources, and link eligible participants to those events.
  • Identify partners to provide health services and screenings to participants at home.
  • Assist participants in building informal support networks with other participants, peers, family, and friends.
  • Engage volunteers within the community or property when appropriate.
  • Conduct individual service coordination through a person-centered, non-clinical assessment, intervention, and monitoring process.
  • Provide program education and enrollment outreach to eligible individuals and regularly engage participants to identify needs and make referrals.
  • Conduct telephone and in-person participant assessments and screenings according to program requirements and established time frames.
  • Collaborate and communicate appropriate information with care and service colleagues to achieve participant goals.
  • Provide customer service by answering questions, addressing concerns, and assisting with basic needs from a person-centered perspective.
  • Assist participants in acquiring and using community services addressing social determinants of health, including housekeeping, meals, transportation, personal services, financial assistance, adult day care, and counseling.
  • Follow participant-preference and program-required monitoring schedules, including telephone and in-person home visits as directed by protocols or needs.
  • Gather reports, participate in interdisciplinary meetings, and support grants or special programs benefiting participants.
  • Build rapport with participants while maintaining professional boundaries.
  • Manage time independently, complete tasks and documentation within identified time frames, and work autonomously and as part of a team.
  • Respond to phone, email, fax, and other communications in a timely and professional manner.
  • Record labor time accurately according to policy and procedure.
  • Complete timely electronic and written documentation of participant interactions, assessments, and services.
  • Complete required forms for participants using the Service Coordination program.
  • Complete care plans and monitor them with participants, including following up on referrals and services.
  • Submit documentation required for funder and quality performance reports.
  • Maintain the required caseload ratio for assigned units or individuals based on scheduled hours.
  • Report participant complaints, abuse allegations, misappropriation of funds or property, and other compliance concerns to appropriate supervisors.
  • Advocate for participants' preferences and right to self-determination.
Desired Qualifications
  • A bachelor's degree in social work or a related field is preferred.
