Complex conditions like COPD and congestive heart failure (CHF) are incredibly challenging to manage. People who are already struggling with isolation and the burden of these conditions become increasingly frustrated and disengaged, eventually falling through the cracks and landing in the ER. This cycle perpetuates poor health outcomes and excessive medical costs.

What you’ll be responsible for:
• Interacting and monitoring about 20 patients per day with a focus in pulmonary and primary care.
•Performing Injections
• Remote assessment and treatment of patients with COPD, asthma, and comorbid conditions including congestive heart failure
• Collaborating with a team consisting of LPNs, PA’s, and a respiratory therapist.
• Communicate with patients to understand their case and provide education to help improve outcomes
• Maintain high patient satisfaction scores
• Maintain timely accurate entry of patient interactions in the EMR
• Supervise the performance and review of remote spirometry
• Participate in continuous process improvement and contribute to innovation
This might be a good fit if you have:
•Bachelors degree
• Current License Nurse Practitioner in TN
• Monday-Thursday to start. Then Monday-Friday starting around fall time
• Hours 7:45am-5pm (about)
•Interested in local travel 2x/month to nursing homes
