Requirements
  • Graduate from an accredited School of Nursing.
  • Hold a New Jersey Registered Nurse license issued by the New Jersey State Board of Nursing in accordance with NJSA 45:11-23 et seq and NJAC 13:37.
  • Have at least one year of Registered Nurse experience.
  • Have strong assessment and problem-solving skills.
  • Be skilled in organization and time management and have knowledge of case management processes.
  • Hold a valid driver's license and have an insured automobile in good working order.
  • Communicate effectively both verbally and in writing.
  • Be fluent in English and Spanish.
Responsibilities
  • Accept an assigned patient caseload from the Patient Care Coordinator or designee.
  • Assume primary responsibility for the assessment, planning, implementation, and evaluation phases of the nursing process for the assigned patient caseload.
  • Communicate with attending physicians, hospice staff members, and other agencies to coordinate optimal care and resource use for patients and their loved ones or caregivers.
  • Maintain regular communication with the attending physician concerning patients and their loved ones or caregivers.
  • Seek input from other team members regarding patient care plans.
  • Provide accurate and timely documentation in electronic medical records.
  • Participate in and contribute relevant information to Interdisciplinary Group meetings.
  • Coordinate the patient's physical care by teaching primary caregivers, volunteers, and employed caregivers, or by providing direct care as appropriate.
  • Meet regularly with hospice nursing staff to review caseload problems or unique issues, provide professional support, and exchange feedback supporting professional growth.
  • Attend patient care conferences and facilitate discussion of caseload issues for staff discussion, consultation, and evaluation.
  • Supervise and document NAC visits every two weeks or as assigned.
  • Inform the Patient Care Coordinator of unusual or potentially problematic patient or family issues.
  • Serve as the associate nurse for patients of other primary nurses during on-call periods or when requested to cover patient or family needs during another nurse's absence.
  • Provide appropriate support at the time of death and during bereavement.
  • Participate in hospice orientation and in-service training programs for professional staff.
  • Participate in agency and community programs as required to promote professional growth and understanding of hospice care.
  • Maintain familiarity with agency policies and the rules and regulations governing hospice care established by state and federal bodies.
  • Accept other assignments as appropriate.
