Requirements
  • A bachelor's degree in a health-related field, counseling, psychology, or social work, or a license as a practical nurse or registered nurse is required.
  • At least one year of relevant experience is required if the bachelor's degree is not in counseling, psychology, or social work or if the employee is not a registered nurse.
  • A valid Tennessee Class D driver's license and state-mandated minimum insurance coverage are required.
  • The driving record must meet Covenant Health minimum standards at the date of hire and throughout employment.
  • Practical nurses and registered nurses must be licensed in Tennessee.
  • The employee must be able to work with people with mental illness and their families in the community.
  • The employee must be able to manage stress in a changing environment.
  • Knowledge of community resources and the ability to develop resources where none are apparent are required.
  • A basic understanding of psychotropic medications and their effects and side effects is required.
Responsibilities
  • Meet with the entire team once weekly to review consumers' treatment needs.
  • Contribute to the development of comprehensive care plans for designated service participants.
  • Work alongside service participants and their families, when indicated, in their natural environments to help them understand, acquire, and maintain needed health care services and develop healthy lifestyles.
  • Plan and assist consumers with activities of daily living, including effective communication, daily routines, functioning in an educational setting, acquiring employment when age appropriate, obtaining legal assistance and counseling, and communicating with medical providers and public agencies such as the police, health department, and courts.
  • Assess consumers' needs across living arrangements, financial, vocational and educational, health, social support, leisure and recreational, and emotional and mental health domains.
  • Assess needs and assist service participants in developing independent-living skills and navigating the service delivery system.
  • Assess needs and assist service participants with planning for and learning social and recreational skills to improve interactions with family and community members.
  • Assess, teach, and assist service participants in job-seeking skills and other employment-related factors.
  • Assess and assist service participants during crises to facilitate re-stabilization.
  • Assess needs and assist service participants in establishing and building support systems and resources.
  • Provide ongoing linkage, monitoring, planning, evaluation, and advocacy for team consumers.
  • Participate in inpatient treatment planning when a consumer is hospitalized to ensure continuity of care.
  • Assess needs and assist service participants in obtaining medical, dental, and other personal care services.
  • Assess eligibility and assist consumers in obtaining financial entitlements, including Supplemental Security Income and Social Security Disability Insurance, and other eligible programs.
  • Attend required staff meetings, training sessions, treatment teams, and other directed meetings.
  • Participate in quality-improvement initiatives.
  • Participate in the on-call rotation.
  • Travel locally as required.
  • Perform other related duties as assigned.

KNOXVILLE, Tenn., Dec. 6, 2024 /PRNewswire/ -- Each of Covenant Health's nine acute-care hospitals in East Tennessee has earned an 'A' hospital safety grade for Fall 2024 from The Leapfrog Group, marking the second consecutive semester all facilities achieved this top rating. The Leapfrog Group collects and reports data related to safety, quality, and other aspects of hospitals' performance. Twice a year, Leapfrog assigns safety grades based on over 30 measures of errors, accidents, injuries and infections, as well as the systems hospitals have in place to prevent them.
