Requirements
  • The candidate must be a registered nurse with a current unrestricted multistate compact license or be willing to pursue the required license.
  • Three or more years of nursing experience are required.
  • Two or more years of case management and/or utilization management experience are required.
  • The candidate must have or be willing to obtain a certification in case management or managed care nursing within two years, if applicable.
  • The candidate must be at least 18 years old at the time of hire.
  • The candidate must have advanced proficiency with Microsoft Word, Excel, and Outlook.
  • The candidate must be able to use multiple software programs simultaneously and type at least 35 words per minute.
  • The candidate must comply with Health Insurance Portability and Accountability Act and protected health information requirements and call-compliance rules and regulations.
Responsibilities
  • Perform within the licensed scope of nursing practice under the direction of the Chief Medical Director.
  • Identify, screen, and enroll eligible members into Care Management Services.
  • Educate members and providers about Medi-Share provisions and processes.
  • Confirm whether members' conditions and plans of care are eligible for sharing using Medi-Share and Food and Drug Administration and Centers for Medicare & Medicaid Services approved evidence-based clinical management guidelines.
  • Identify, educate, and facilitate member participation in stewardship resources, including patient assistance programs for medications, laboratory services, and genetic testing.
  • Provide disease- and condition-specific clinical education, including multimedia resources when available.
  • Use appreciative inquiry, motivational interviewing, and change-management coaching skills to facilitate improved clinical outcomes.
  • Identify opportunities for and facilitate transitional care.
  • Partner with organizational resources to facilitate effective and cost-efficient member outcomes.
  • Deliver accurate and comprehensible verbal and written communication to external customers while complying with organizational HIPAA requirements.
  • Document case work, telephone conversations, correspondence, and tasks completely and accurately in appropriate information-technology systems.
  • Independently manage the assigned caseload and departmental workflows within agreed service levels.
  • Present accurate, complete, and concise case information to the Senior Director of Clinical Services within agreed service levels.
  • Complete medically related special projects independently with minimal supervision.
  • Attend required departmental and organizational meetings.
  • Pray with members and providers as appropriate.
  • Contribute to the exercise and expression of the ministry's Christian beliefs.
  • Perform other assigned duties.
Desired Qualifications
  • A Bachelor of Science in Nursing is preferred.
  • Acute and catastrophic care experience is highly preferred.
  • Adult oncology experience is strongly preferred.
  • Transitional care and transplant clinical experience are highly preferred.
