Requirements
  • A minimum Bachelor's degree in Social Work, Psychology, Sociology, or another field related to social work is required.
  • One year of social work experience in a healthcare setting is required.
  • A minimum of two years of experience providing grief, loss, and/or bereavement support to individuals and families is required.
  • Licensure is required if mandated by state rules; in Michigan, acceptable licenses include Licensed Bachelor's Social Worker, Licensed Master’s Social Worker, Licensed Clinical Social Worker, or Licensed Independent Clinical Social Worker.
  • The role requires supervisory oversight by a Master of Social Work for hospice patient care planning and counseling needs.
  • The team member must be able to add, subtract, multiply, and divide using whole numbers, common fractions, and decimals, and compute rates, ratios, and percentages.
  • The team member must be able to read, analyze, and interpret business periodicals, professional journals, technical procedures, and governmental regulations.
  • The team member must be able to write reports, business correspondence, and procedure manuals.
  • The team member must be able to present information and respond to questions from leaders, team members, investors, and external parties.
  • The team member must understand, read, write, and speak English.
  • The team member must have strong organizational and time-management skills.
  • The team member must be able to work a flexible schedule including nights and weekends.
  • The role requires regular standing, walking, manual dexterity, and the ability to lift and move objects up to 50 pounds.
  • The role requires visual acuity for close and distance vision, color and peripheral vision, depth perception, and adjusting focus.
  • The role requires routine use of standard medical equipment and tools associated with clinical care, with expected exposure to bodily fluids and infectious diseases.
Responsibilities
  • Explain hospice services and Medicare benefits, and obtain required consents and documentation.
  • Complete and submit documentation in a timely manner according to company policy and regulatory requirements.
  • Read and apply company policies, procedures, and regulatory guidelines when providing care and services.
  • Participate as a member of the interdisciplinary team and contribute to developing and implementing the plan of care.
  • Provide psychosocial and bereavement support to patients, families, and caregivers, and engage external or community resources as appropriate.
  • Develop, update, and maintain care plans reflecting current problems, goals, and interventions.
  • Conduct initial and ongoing psychosocial assessments addressing financial resources, insurance, living arrangements, caregiver stressors, anticipatory grief, and community resource needs.
  • Facilitate long-term-care placement when needed and participate in care-planning meetings at facilities.
  • Support and guide advance-care planning, including living wills, power-of-attorney documentation, funeral arrangements, memorial services, and body or organ donation.
  • Supervise social-work practicum students as applicable.
  • Promote and assess bereavement risk factors at admission and throughout patient care, and develop and implement bereavement care plans after the patient's death.
  • Provide or coordinate grief counseling, support groups, memorial services, and community education related to bereavement.
  • Identify and support the bereavement needs of non-family caregivers, including staff and facility residents.
  • Collaborate with the Volunteer Coordinator to integrate volunteer support into bereavement activities.
  • Maintain a library of bereavement resources and provide appropriate materials to patients, families, and caregivers.
  • Prepare reports, in-services, and other documentation demonstrating the value and effectiveness of the bereavement program.
  • Attend and participate in community events and public outreach promoting bereavement support services.
  • Perform other duties as assigned.
Desired Qualifications
  • A Master's degree in Social Work is highly preferred, with one year of social work experience in a healthcare setting.
  • Licensure, if required by state rules.
  • Clinical experience or training related to death, dying, and bereavement.

Compassus appoints Chris Core as chief growth officer to lead home-based care expansion
