Requirements
  • Possess, at minimum, a Nursing Degree from an accredited college or university, or graduate from an approved Licensed Practical Nurse/Licensed Vocational Nurse program.
  • Possess a current, unencumbered, active license to practice as an LPN/LVN in the state.
  • Read, write, speak, and understand the English language.

Responsibilities
  • Direct the day-to-day functions of nursing assistants in accordance with rules, regulations, and long-term-care-facility guidelines.
  • Ensure that assigned nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Review department policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident's total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys conducted by authorized government agencies.
  • Review, update, and assist with nursing services portions of resident discharge plans.
  • Admit, transfer, and discharge residents as required.
  • Complete accident and incident reports, maintain required records, and submit reports to the Nurse Supervisor or Director.
  • Maintain daily census reports and forward resident charge slips to the Business Office.
  • Complete medical forms, evaluations, studies, charting, and other administrative documentation.
  • Protect user credentials and resident protected health information and report suspected unauthorized access or disclosure.
  • Complete admission, transfer, discharge, charting, medication, treatment, and care-plan documentation.
  • Receive telephone orders from physicians and transcribe physician orders to resident records, medication cards, treatment plans, and care plans.
  • Chart nurses' notes informatively and descriptively to reflect care provided and resident responses.
  • Prepare and administer medications as ordered by physicians and verify resident identity before administration.
  • Maintain adequate supplies, equipment, and floor-stock medications and report needs to the Nurse Supervisor.
  • Order prescribed medications, supplies, and equipment in accordance with facility policies.
  • Maintain accurate narcotic records, report discrepancies, review medication cards, and follow automatic stop-order and drug-disposal procedures.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend employee dismissals or transfers to the Nurse Supervisor.
  • Report staffing shortages and absences and recommend workforce actions.
  • Develop work assignments and provide leadership to nursing personnel assigned to the unit or shift.
  • Make daily rounds to ensure nursing personnel perform assignments according to acceptable nursing standards.
  • Meet regularly with nursing personnel to identify and correct problem areas and improve services.
  • Ensure personnel, residents, and visitors follow departmental policies and procedures.
  • Maintain effective working relationships across departments and review assigned personnel complaints and grievances.
  • Administer departmental disciplinary action fairly and without discrimination.
  • Give and receive nursing reports at the beginning and end of shifts.
  • Report occupational exposures, suspected fraud, and information-system security incidents.
  • Inform nursing personnel about new admissions and ensure rooms are ready for admissions.
  • Greet and orient new residents and family members to the facility.
  • Make physician rounds, arrange diagnostic and therapeutic services, and consult physicians regarding resident care.
  • Review resident charts for treatments, medications, diets, and other orders.
  • Implement and maintain nursing objectives and standards.
  • Check that prescribed treatments are properly administered and evaluate residents' physical and emotional status.
  • Provide or ensure direct nursing care by qualified licensed nurses, certified nursing assistants, or nurse aide trainees.
  • Coordinate social and activity programs with nursing schedules.
  • Notify physicians and next of kin of resident accidents, incidents, or changes in condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Inspect treatment areas daily and maintain clean and safe nursing-service areas.
  • Perform professional nursing services including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, blood collection, massage, range-of-motion exercises, and care of dying residents.
  • Use restraints when necessary and according to facility procedures.
  • Collect sputum, urine, and other specimens for laboratory testing.
  • Take and record temperature, pulse, respirations, blood pressure, and other measurements.
  • Monitor seriously ill residents and check residents who cannot call for help.
  • Review food brought by residents' families or visitors for compliance with dietary allowances.
  • Ensure direct-care personnel follow resident care plans and wishes.
  • Meet with residents and family members and report problems to the Nurse Supervisor.
  • Arrange transportation for discharged residents and escort discharged residents to the pickup area.
  • Inform family members of resident deaths, contact funeral homes when requested, and ensure post-mortem procedures are followed.
  • Develop, plan, conduct, and schedule in-service training classes.
  • Implement and maintain an orientation program for new employees.
  • Standardize work methods and train personnel on tasks involving potential exposure to blood or body fluids.
  • Assist with clinical supervision planning for nurse aide trainees.
  • Attend outside training, annual facility in-service programs, advance-directive training, and continuing education.
  • Monitor assigned personnel for compliance with safety regulations, dress codes, hand hygiene, infection control, fire safety, and disaster preparedness procedures.
  • Ensure appropriate personal protective equipment is available and used for procedures involving blood or body-fluid exposure.
  • Participate in developing, implementing, and maintaining infection-control programs and hazardous-condition reporting procedures.
  • Ensure personnel use safety equipment, operate equipment safely, and follow procedures for hazardous chemicals and material safety data sheets.
  • Recommend unit equipment and supply needs and maintain adequate medication, medical-supply, and equipment stock levels.
  • Participate in developing and implementing procedures for safe operation of nursing-service equipment and efficient use of supplies.
  • Review resident care plans daily and notify the Nurse Supervisor of needed changes.
  • Ensure nurses' notes reflect care-plan implementation and review care plans for appropriate goals, problems, approaches, and revisions.
  • Ensure certified nursing assistants know and follow resident care plans.
  • Assist the Resident Assessment/Care Plan Coordinator with scheduling, revising, and implementing the Minimum Data Set, Resident Assessment Protocols, and triggers.
  • Provide data to the Quality Assurance and Assessment Committee as requested.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego.
