Responsibilities
  • Coordinate and participate in activities related to care management services for family members and caregivers.
  • Use patient-centered motivational interviewing techniques to build rapport and help patients improve their health.
  • Support the primary care team through panel management to reduce the number of patients lost to care, noncompliant with follow-up care, or disconnected from primary care.
  • Participate in developing, maintaining, and adjusting individualized care plans for high-risk patients addressing medical and social barriers to care access.
  • Act as a professional liaison among hospitals, primary care providers, specialists, and community resources to ensure patient-centered care coordination.
  • Identify and track special populations, including high-risk patients and populations due for preventive or chronic care services.
  • Identify and track patients discharged from inpatient services or the emergency department.
  • Use team-based communication strategies to close the loop on referrals, hospital follow-ups, and outstanding items in patient care plans.
  • Perform outreach activities at primary care sites, homes, hospitals, and in neighborhoods.

Dr. Saint will serve as chair of the Department of Medicine at Long Island Jewish (LIJ) Medical Center in New Hyde Park, North Shore University Hospital (NSUH) in Manhasset and the Donald and Barbara Zucker School of Medicine at Hofstra/Northwell in Hempstead, where he will be appointed the endowed David J. It is important for a CMO and other senior leaders to gain institutional buy-in for an innovation in critical care, which involves ensuring that an innovation is evidence-based, that it has been shown to improve patient outcomes, and that it is either cost-neutral or capable of decreasing costs. An innovation in critical care should be attainable, with considerations such as whether an institution can support the innovation and whether there is a return on investment. Samuel Acquah, MD, who became the new vice president and CMO of Northwell Health's Phelps Hospital in July, has extensive experience in critical care innovation. Acquah's prior leadership experience includes serving as section chief of medical critical care at Mount Sinai Health System. The keys to success in critical care innovation include choosing innovations that have been shown to improve patient outcomes and to improve operational excellence, according to Acquah. "You also should have a clear vision of how an innovation aligns with the priorities of your health system or hospital," Acquah says. "For example, an innovation in critical care should have an impact on patient safety, mortality, and decreasing costs." Ensuring that an innovation is aligned with an organization's priorities includes assessing the needs of an institution and any gaps in care, Acquah explains. "Once you identify those needs and gaps, then you must determine whether an innovation can meet those needs and close those gaps," Acquah says. It is important for a CMO and other senior leaders to gain institutional buy-in for an innovation in critical care, which involves ensuring that an innovation is evidence-based, that it has been shown to improve patient outcomes, and that it is either cost-neutral or capable of decreasing costs, according to Acquah. Acquah was a physician champion in the adoption and implementation of a veno-venous extracorporeal membrane oxygenation (ECMO) program at Mount Sinai's medical ICU. Adoption of the veno-venous ECMO program reflected Acquah's philosophy on clinical care innovation. "No. 1, we realized there was a need for a veno-venous ECMO program," Acquah says. "Once that determination was made, I made sure the program was attainable." Ensuring the program was attainable included factors such as what the institution could support and the potential for return on investment. From an institutional support perspective, the ability to form a multidisciplinary team for the veno-venous ECMO program was crucial. "A veno-venous ECMO program requires a multidisciplinary team, including the involvement of pulmonologists, cardiothoracic surgeons, perfusionists, respiratory therapists, and well-trained nurses," Acquah says. "A veno-venous ECMO program also needs support from key hospital departments such as cardiology and lung transplant." Senior leaders believed the veno-venous ECMO program would improve the health of patients, which would drive ROI, Acquah says. Once the ability to form a multidisciplinary team to support the program was established and there was an expectation of a return on investment, a focal point of implementation was simulation training. "Simulation has become an important element of critical care training," Acquah says. "To have a successful simulation program, you need physicians who are well-versed in simulation training, you need to ensure that training participants understand that it is safe to fail during a simulation exercise, and you need to make sure that simulation scenarios have a particular purpose." There are two considerations for ensuring that simulation scenarios are purpose-driven, according to Acquah. "Most simulation scenarios focus on low-frequency but high-acuity events," Acquah says. "These are events that do not happen often but can have catastrophic consequences when they occur."

Twenty-one Northwell Health hospitals recognized by American Heart Association for heart and stroke care. Twenty-one Northwell Health hospitals earned 2026 Get With The Guidelines achievement awards from the American Heart Association, recognizing quality and consistency in treating stroke, heart attack, heart failure, and cardiac arrest, according to an Aug. 20 announcement. The annual awards honor hospitals that follow the American Heart Association's evidence-based treatment guidelines and demonstrate a commitment to improving clinical outcomes. Of the recognized facilities, 18 received Gold Plus status for stroke care. Several hospitals also achieved additional distinctions for rapid stroke treatment and advanced therapy. North Shore University Hospital, South Shore University Hospital, Phelps Hospital, and Norwalk Hospital earned Stroke Gold Plus recognition with Target: Stroke Elite Plus distinctions. Additionally, North Shore University Hospital, South Shore University Hospital, and Norwalk Hospital were awarded Target: Stroke Advanced Therapy recognition. Lenox Hill Hospital, Long Island Jewish Medical Center, Staten Island University Hospital-North and Prince's Bay campuses, Mather Hospital, among others within the Northwell Health system, received Stroke Gold Plus recognition with Target: Stroke Elite Honor Roll distinctions.
