Responsibilities
  • Delivers basic patient care under the guidance and direction of the Registered Nurses and Licensed Practical Nurses on the unit to safely meet the needs of the patient(s); the CNA refers to the patient care checklist to maintain patient care goals and communicate with the RN/LPN to coordinate workload and patient priorities
  • Receive report, prioritize patients that are critically ill, difficult, or scheduled for surgery first
  • Maintain 24 Hour Intake and Output flow sheets for every patient
  • Record vitals in appropriate documentation and inform RN/LPN of any abnormalities
  • Basic CNA assessment, inform RN/LPN of any change in patient status
  • Recording intake after meals
  • Answer call lights and phones throughout shift
  • Chart in CNA flow sheets for patient(s)
  • Keep patients hydrated, ensuring fresh water is available at the end of every shift
  • Collect specimen(s) and deliver to lab
  • Empty output equipment throughout shift (Urinals, commodes, foleys, gastric suction, and catheters)
  • Give AM/PM care to patient(s) that request
  • Meet patient personal hygiene needs and goals; assist with activities of daily living such as ambulation, transferring to chair, eating and grooming
  • Monitor patient activity to prevent harm; remove obstacles as part of fall prevention protocol
  • Clean up after meals and return dishware to Long-Term care facility
  • Maintain clean patient area(s) including patient rooms
  • Empty trash, clean bathroom, stock fresh linen, make beds, remove equipment not being used
  • Bag dirty laundry and transport to designated area
  • Keep utility and stock room neat and free from debris
  • Clean and stock Dietary kitchen
  • Defrost and clean refrigerator in Dietary Kitchen as needed
  • Guide and/or assist patient to the appropriate room
  • Perform initial intake duties to include weight and vitals
  • Fill out clothing intake sheet and have patient sign for their record
  • Have patient sign appropriate form(s) if not done at Admissions Office: Patient Rights; Privacy Practice
  • Demonstrate the ability to maintain professional relationships with peers, supervisors, physicians, and patients
  • Maintain patient care, respond to changing work priorities, and communicate professionally and respectfully
  • Adhere and uphold Logan Health customer service policies and standards of care
  • Maintain performance and attendance in a consistent manner as assigned by Nursing Administration

Logan Health Shelby receives Cardiac Recognition Award. Logan Health Shelby was recognized recently with the Cardiac Recognition Award, which is sponsored by the Cardiovascular Health Program within the state health department. The Recognition Award singles out critical access hospitals who demonstrate that they have the infrastructure and commitment to provide high quality care for patients with acute coronary syndrome, which includes heart attack. Receiving the Cardiac Recognition confirms Logan Health Shelby has met high standards related to the utilization of evidence-based cardiac order-sets and protocols, provides laboratory services 24/7, has a well thought out transfer protocol and has administrative commitment to treating heart attack patients. This is the 14th year that the Cardiac Recognition Award was presented, and Logan Health Shelby is one of 20 Critical Access Hospitals to qualify for the award. Requirements for the recognition include tracking and reporting key cardiac outcomes, using outcomes data to initiate quality improvement activities and coordinating regular healthcare provider cardiac trainings. Heart disease is a leading cause of death in both Montana and the United States. Last year, over 3,000 Montanans died from the impact of heart disease.
