- LPN credential.
- Completion of Skin Health Team Lead orientation training within 90 days of hire.
- Completion of Wound Basics within 90 days of hire.
- Completion of Advanced Wound Training and a nationally accredited wound certification within one year of hire; candidates hired with wound certification must complete a fast-track hybrid program for regulatory wound-care requirements.
- Assist in developing and facilitating a person-centered team approach to patient and resident care related to wound prevention and management.
- Report directly to a designated Skin Health Team Lead and the Director of Nursing, with a dotted-line reporting relationship to the Nursing Home Administrator.
- Collaborate with the interdisciplinary team to communicate with family members, responsible parties, authorized representatives, and patients and residents about skin and wound prevention and management, changes in condition, and the overall residency experience.
- Coordinate and lead weekly team wound rounds and post-meeting follow-up discussions.
- Train staff in aseptic technique, wound measurement and evaluation, wound-type recognition, skin checks, turning and positioning, treatments, and interventions.
- Communicate with appropriate staff and coordinate readiness for potential patient and resident admissions involving skin or wound needs, together with the designated Skin Health Team Lead and Director of Nursing.
- Serve as the bed and chair surface specialist, negative-pressure wound therapy resource, and incontinence and ostomy resource.
- Act as a clinical expert for skin, wound, and incontinence products and provide oversight for durable medical equipment related to wound prevention and management, including support surfaces and negative-pressure equipment.
