Requirements
- Licensed social worker with 7–10 years of experience working in geriatrics.
- Ability to work with minimal supervision.
- Valid driver's license and dependable personal vehicle.
Responsibilities
- Manage a small caseload of private-pay clients, including clients residing in independent living, assisted living, or nursing facilities.
- Coordinate services, resources, and ongoing monitoring and management for clients in their homes or senior communities.
- Develop relationships with community and professional organizations to generate client referrals.
- Perform in-depth client assessments covering medical, psychosocial, environmental, financial, legal, and family factors.
- Complete a Fall Risk Assessment, Mini-Mental Status Examination, and depression screening for each client.
- Recommend and coordinate all service needs for clients.
- Work with clients and families to develop plans of care that address safety and other needs.
- Work with clients, families, and professional staff to implement plans of care.
- Collaborate with hospitals, nursing homes, assisted living and memory-care staff, physicians, medical providers, attorneys, financial planners, Meals on Wheels, and other geriatric-service professionals.
- Perform regular home visits to monitor client status and the home or senior-community environment.
- Provide emotional support and counseling to clients and their families as needed.
- Provide problem-solving support to clients and fellow care managers.
- Provide crisis intervention and apply critical-thinking and problem-solving skills.
- Work with the business-development team to meet prospective clients, families, and referral sources.
- Provide backup support to the Geriatric Care Management team as needed.
- Track and document time spent on each client to facilitate accurate billing.
- Attend educational programs and networking events.
