Position: Behavioral Health Provider
Status: 40 hours/week full-time and benefits eligible. Must be available to work all hours the clinic is open as schedule will vary.
Primary Purpose: Work as part of the medical home at West Hills Healthcare Clinic with medical patients to mitigate the behavioral barriers impacting their medical and psychosocial functioning. Behavioral Health Provider will accomplish this by completing an initial assessment, including population-based screeners and brief interventions. They will support the provider team by offering consultation and support on behavioral health issues. This position has a collaborative relationship with the continuum of services in the Yamhill County Care Organization, including the Behavioral Health Provider Learning Collaborative that meets monthly to achieve the standards established by the YCCO.
Education and/or Training:
Must have appropriate credentials and level of training.
Appropriate credentials include:
Licensed Clinical Psychologist or working towards licensure (post-doctoral candidate)
Master’s degree in social work and licensed as an LCSW
Licensed Professional Counselor
Applicant must be licensed in the state of Oregon.
Must have graduate course work in biological basis of behavior, understanding of biopsychosocial model of health, theory, diagnosis, and treatment of mental health and dual diagnosis disorders.
Experience working with people who have experienced chronic medical conditions.
Demonstrate proficiency using DSM V, and ICD-10 and ability to document appropriately in an electronic health record.
Bilingual preferred.
Work Environment:
This job operates in a professional office environment. This role routinely uses standard office equipment such as: computers, phones, photocopiers, and fax machines.

Physical Demands: While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit, stand, walk; use hand to finger, and reach with hands and arms. This position requires the ability to occasionally lift office products and supplies weighing up to 5-20 pounds. Essential Functions and Duties:
- The Behavioral Health Provider will see a patient for an average of 1 to 5 times based on 30- minute schedule increments. A portion of these patients will be ' high utilizer' patients and other patients identified through the scrubbing and referral processes. High utilizer patients are those with chronic health conditions who have been identified as using the ER excessively or inappropriately. Identified patients are experiencing negative outcomes with their chronic health conditions and can benefit from the use of brief therapy to assist them with behavioral interventions that could improve their outcomes, increase their self-management skills, and reduce behavioral barriers to wellness.
- Provide information, screening, and referral services to patients who would benefit from additional services provided in Yamhill County.
- Assess the nature and extent of each patient's emotional, behavioral, mental health, and/or co- occurring disorders, and provide any brief interventions necessary, typically ranging from 1-5 face to face visits (with some ongoing telephonic outreach) to ensure behavioral barriers to appropriate care are reduced.
- Incorporate the use of the SBIRT, PHQ, and other screening tools into his/her interventions and collaborate and communicate with team members to provide integrated treatment.
- Work with Health Educators, Providers, Back Office Care Coordinators, and other Team Members to provide continuity of care for patients with complex cases.
- Receive warm handoffs from providers in accordance with integrated care model that creates accessibility in schedule and facilitates a warm handoff to Mental Health when possible and ensure coordination of services.
- Participate in clinical review sessions, monthly staff meetings, training sessions, committee work, supervision sessions, including Behavioral Health Provider Learning Collaborative.
- Demonstrate awareness of co-occurring disorders and chronic conditions and how they impact patient outcomes and care.
- Establish rapport, systematically gather data, determine the patient’s readiness for treatment and change, and apply accepted criteria for diagnosis of substance abuse disorders.
- Close collaboration with our internal metrics team to impact reimbursements for value-based care.
- Screen for psychoactive substance toxicity, intoxication, withdrawal symptoms, aggression or danger to others, potential for self-inflicted harm or suicide, and coexisting behavioral health problems.
- Analyze and interpret all relevant data to determine treatment recommendations and priorities.
- Provide crisis intervention and help deescalating difficult or upset patients.
- Receives warm handoffs from medical providers for BH interventions related to medical problems such as insomnia, diabetes, hypertension, headaches, pain, obesity and weight management, asthma, smoking cessation, somatization, compliance with disease-specific medical regimen.
- Collaborate with Mental Health Providers on providing wrap around services to shared patients and those needing behavioral support.
