Requirements
  • A current Licensed Practical Nurse license in the state of employment is required.
  • At least one year of experience as a Licensed Practical Nurse in an acute care setting is required.
  • The ability to communicate effectively with patients, families, physicians, and co-workers.
  • The ability to remain calm, patient, and accommodating.
  • Compassion and care when working with patients.
  • Knowledge of nursing best practices.
  • The ability to make appropriate nursing judgments.
  • The ability to identify situations and handle them with the best possible solution.

Responsibilities
  • Provide skilled nursing care in patients' homes as ordered by the attending physician.
  • Perform care by following order sets and goals established by the registered nurse.
  • Observe and monitor patient conditions.
  • Notify the registered nurse case manager of changes in patient condition.
  • Administer medication as prescribed by the physician.
  • Counsel patients and families on disease or injury processes and management.
  • Incorporate patients and families in developing the plan of care.
  • Teach patients preventative measures and sound decision-making to help decrease rehospitalizations.
  • Listen to patients and family members.
  • Complete clinical documentation in accordance with agency protocol and Medicare and federal guidelines.
  • Document treatment, assessments, and patient education.
  • Maintain an active Licensed Practical Nurse license in the state of employment.
  • Communicate schedule changes outside the normal schedule to the scheduler.
  • Participate in on-call requirements and case conferences.
  • Attend in-service training and mandatory agency meetings.
  • Maintain a professional appearance as a company representative.
  • Follow agency policies and procedures.
  • Drive to patients' primary locations for scheduled visits.
  • Attend assigned visit locations daily.
  • Document subsequent visits within 24 hours to meet Medicare guidelines.
  • Complete and submit required documentation within company deadlines.
  • Provide skilled care within the Licensed Practical Nurse scope of practice.
  • Fulfill Licensed Practical Nurse visits in the assigned geographic location according to patient need and federal and state specifications.
