Requirements
  • A Bachelor of Science in Nursing and three years of clinical experience in a healthcare setting with responsibilities reflecting direct management of patient care, including planning, coordination, and delivery of services, or an Associate’s Degree in Nursing and five years of such clinical experience.
  • A current Registered Nurse license issued by the state in which services will be provided or a current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact.
  • Demonstrated computer literacy.

Responsibilities
  • Assist in the development and implementation of quality activities and monitor quality indicators for assessment of quality across ambulatory and inpatient care.
  • Develop and implement mechanisms for ongoing data collection, analysis, and reporting of quality indicators, ensuring timely and accurate reporting.
  • Monitor appropriate criteria and report data for specific procedures and services.
  • Develop performance improvement initiatives throughout the institution and support sustained performance improvement.
  • Work collaboratively with Decision Support personnel and staff for variance tracking.
  • Collaborate with the interdisciplinary healthcare team to support resource utilization and quality improvement, using information systems to collect and analyze data.
  • Collect, analyze, and present data via internal software systems, external comparative databases, manual review, survey audits of departments, chart review, and staff interviews.
  • Prepare the institution for Joint Commission standards and Centers for Medicare and Medicaid Services conditions of participation.
  • Analyze data for trends and for medical staff and institutional compliance with Joint Commission standards and Centers for Medicare and Medicaid Services conditions of participation.
  • Develop and submit reports for appropriate individuals and committees within the organization, and present quality initiative updates, reports, and education to faculty and staff.
  • Assist in developing policies, processes, and activities supporting continuous readiness for Joint Commission and Centers for Medicare and Medicaid Services requirements.
  • Monitor patients’ care for compliance with Joint Commission Core Measures and Centers for Medicare and Medicaid Services Scope of Work initiatives.
  • Assist in implementing and monitoring evidence-based indicators associated with Joint Commission Core Measures and Centers for Medicare and Medicaid Services Scope of Work initiatives.
  • Develop and implement mechanisms for ongoing monitoring and analysis of evidence-based indicators.
  • Monitor appropriateness criteria and alert services to patient-centered opportunities.
  • Document evidence of compliance or exclusion criteria for evidence-based indicators in the medical record.
  • Develop performance improvement initiatives related to evidence-based indicators and support sustained performance improvement.
  • Analyze data for trends and compliance with Joint Commission Core Measures and Centers for Medicare and Medicaid Services Scope of Work initiatives.
  • Interact with faculty and staff to attain high levels of compliance with Joint Commission Core Measures and Centers for Medicare and Medicaid Services Scope of Work initiatives.
  • Assist in developing policies, processes, and activities supporting compliance with Joint Commission Core Measures and Centers for Medicare and Medicaid Services Scope of Work initiatives.
  • Participate with Insurance Services in payor-based quality improvement initiatives, including QualityBLUE and Ambulatory QualityBLUE, and present findings, initiatives, and recommendations to payors.
  • Provide and participate in patient education and disease management in conjunction with Centers for Medicare and Medicaid Services, QualityBLUE, Get With The Guidelines, and other best practices and initiatives.
  • Interact with and educate patients on disease states, medical management, and possible adverse events, including congestive heart failure and anticoagulation.
  • Collaborate with physicians, nursing staff, and other healthcare providers to ensure comprehensive, coordinated patient and family education.
  • Promote excellence in disease state management education by modeling advanced skills and providing instruction and expertise for complex problems.
Desired Qualifications
  • An advanced degree, such as a Master of Science in Nursing, Master of Public Health, or Master of Public Administration.
