Gradia Health provides a virtual platform that enhances the experience of patients with chronic conditions by extending the services of traditional clinics. The platform allows patients to stay updated on their care, communicate easily with healthcare providers, and access specialized virtual care teams. This approach not only aims to improve patient outcomes but also helps clinics increase their revenue by allowing physicians to delegate tasks and provide a higher level of personalized care. Gradia's goal is to make attentive and integrated care the standard for patients while also improving job satisfaction for healthcare providers. Clinics pay a subscription or service fee to use Gradia's platform, which supports better patient management and care delivery.
