Requirements
  • A high school diploma or General Education Degree is required.
  • Six months to one year of related experience is required.
  • The ability to operate a computer and electronic medical records is required.
  • A Michigan Certified Nursing Assistant certification is required.
  • The candidate must complete, or be able to complete, the Green House Shahbazim Program.
  • The candidate must complete, or be able to complete, a Basic First Aid course.
  • The candidate must complete, or be able to complete, a ServSafe course.
  • The candidate must be able to work evenings, weekends, and holidays.
Responsibilities
  • Provide assistance with elders’ activities of daily living, including toileting, incontinence care, bathing, dressing and undressing, life enrichment, and maintaining independence.
  • Provide gentle, empathetic, efficient, timely, dignified, and respectful care based on each elder’s needs, preferences, and desired approach.
  • Ensure each elder’s call device is worn or within reach and answer it promptly.
  • Care for elders and their environment, including cooking, laundry, housekeeping, changing bed linens, and making beds.
  • Recognize and respond to elders’ needs while encouraging independence.
  • Administer medications and treatments as prescribed by a physician.
  • Apply clean dressings, slings, stockings, and support bandages under the direction of a nurse or physician.
  • Measure and record vital signs as requested by the PACE nurse case manager or physician.
  • Document care as required by each elder’s Service Plan.
  • Observe elders for symptoms requiring medical attention, including bruises, open wounds, or blood in urine.
  • Prepare and cook meals and snacks according to nutrition guidelines, dietary restrictions, allergies, and preferences; serve and assist elders with dining.
  • Maintain confidentiality of elder information and care in compliance with HIPAA and Resident Rights.
  • Assist with transportation coordination to the PACE center or other locations using wheelchairs, assisted walking, and lift equipment.
  • Review and implement all aspects of each elder’s Service Plan.
  • Maintain a safe environment by checking elders’ whereabouts and immediately reporting residents who leave the premises.
  • Attend required meetings and training opportunities to remain current on information and skills.
  • Follow Presbyterian Villages of Michigan, safety, and applicable federal, state, and local policies and procedures.
  • Maintain required CNA certification and participate in annual state in-services.
  • Perform other duties as assigned.
