  • A Bachelor of Science degree or Doctor of Pharmacy degree is required.
  • At least one year of pharmacy experience is required.
  • Candidates must be eligible for licensure by the Texas State Board of Pharmacy upon hire and obtain a Texas license with the Texas State Board of Pharmacy within 30 days.
  • Candidates must hold an ACPE-accredited sterile compounding intravenous certification upon hire.
  • Candidates must hold Basic Life Support certification accredited by the American Heart Association or American Red Cross upon hire.
  • Advanced Cardiac Life Support certification accredited by the American Heart Association or American Red Cross is required for inpatient positions within 180 days.
  • Candidates must have basic computer and data-entry skills.
  • Candidates must be able to speak, read, and write English.
  • Candidates must be able to perform prolonged standing and walking, occasional lifting and carrying of up to 25 pounds, reaching, bending, and crouching, and must meet OSHA Category III requirements.
Responsibilities
  • Perform job responsibilities in accordance with organizational and departmental policies, procedures, performance-improvement objectives, environmental and infection-control standards, and state and federal laws and regulations.
  • Complete computer order entry accurately, thoroughly, and promptly; review and update medication profiles, including therapies, allergies, height, and weight, and account for patient charges.
  • Review medication orders for therapeutic appropriateness, duplications, interactions, effectiveness, and safety; seek clarification when necessary and document adverse drug reaction interventions.
  • Work independently with minimal supervision, take charge when necessary, make sound decisions, cross-train, and maintain competency in multiple areas.
  • Enforce the formulary system and follow departmental procedures when acquisition of non-formulary products is necessary; participate in cost-effective pharmaceutical selection and use.
  • Communicate drug information verbally and in writing to physicians, nurses, and other personnel; assist with patient and family education and counseling regarding drug therapy and side effects.
  • Document clinical activities, demonstrate cost savings, and convey key information for follow-up by fellow pharmacists.
  • Manage technicians and other supportive personnel, delegate appropriate tasks, validate medication accuracy for distribution from the intravenous laboratory, central pharmacy, and decentralized automated dispensing machines, and audit and sign out controlled substances.
  • Investigate and correct discrepancies and unusual usage patterns or trends.
  • Participate in educational programs, in-services, meetings, new-employee orientation, training, and additional projects as needed.
  • Participate in the clinical program and support Pharmacy and Therapeutics initiatives by monitoring target drugs for protocol discontinuation, substitution, and intravenous-to-oral conversion.
  • Collect applicable clinical data and measures, communicate improvement opportunities, perform therapeutic monitoring, and recommend dosage adjustments based on renal-function evaluation and pharmacokinetic calculations.
  • Directly interact with or care for patients, research participants, patient records, or human-subjects research records as applicable.
  • Perform other duties as assigned.
