Requirements
  • A Pharmacy degree, specifically a Pharm.D. or Bachelor of Pharmacy, from an accredited school of pharmacy is required.
  • Two years of experience or completion of a Postgraduate Year 1 residency is required.
  • Basic Life Support Healthcare Provider certification is required within 90 days of employment; other resuscitation certifications may be required depending on the department.
  • A pharmacist license in the applicable state of work location is required.
  • Board certification in an accepted pharmacy specialty or Certified Specialty Pharmacist certification is required.
  • The role requires frequent lifting, carrying, pushing, and pulling of objects weighing 0–25 pounds; frequent sitting, standing, walking, reaching, and repetitive foot, leg, hand, and arm movements.
  • The role requires frequent near and far vision, depth perception, color identification, hearing, oral communication, and keyboard use/data entry.
  • The role requires occasional bending, stooping, kneeling, squatting, twisting, and gripping, as well as occasional lifting, carrying, pushing, and pulling of objects weighing 25–50 pounds; rare climbing.
Responsibilities
  • Provide pharmaceutical care in the specialty practice area by reviewing and evaluating patient drug and medical histories, recommending drug therapy, providing drug information, evaluating medical literature, rounding with healthcare providers, conducting drug utilization reviews, counseling patients, and performing order entry, verification, and dispensing.
  • Plan, implement, perform, and measure clinical processes that improve treatment outcomes, minimize adverse drug events, and contribute to quality and financial outcomes.
  • Coordinate drug utilization evaluations, analyze trends, and evaluate data for synthesis into reports for committee action.
  • Create a culture of learning and sharing that supports qualified clinical staff through coaching, motivation, and empowerment.
  • Implement staff development on clinical processes and participate in training pharmacists, pharmacy residents, pharmacy students, and healthcare providers.
  • Promote interdisciplinary teamwork through development and implementation of clinical pathways and protocols that support appropriate medication safety and medication utilization using evidence-based recommendations.
  • Work in a constant state of alertness and in a safe manner.
  • Perform other duties as assigned.
