Requirements
  • A high school diploma or GED is required.
  • At least two years of experience in social service delivery with elderly people, people with disabilities, and/or families is required.
  • Working knowledge of supportive services and other resources in the area served by the project is required.
  • The ability to advocate, organize, solve problems, and provide results for participants is required.
  • Good communication, comprehension, and interpersonal skills are required.
  • Working knowledge of Windows, Microsoft Office Suite, and the World Wide Web is required, demonstrated through a passing computer literacy test.
  • The ability to speak, read, write, and understand English is required.
  • The ability to perform basic arithmetic operations involving money, using a calculator or Excel spreadsheet when needed, is required.
  • Occasional travel or travel directed by contract and department requests may be required.
Responsibilities
  • Coordinate programs and activities for eligible individuals on a group basis.
  • Serve as a liaison to community agencies, network with community service providers, and identify new services and low-cost providers or discounts.
  • Monitor services provided by community agencies and document participant use in the designated documentation management system.
  • Develop and maintain a resource directory of state and local service providers.
  • Organize educational events on health care, agency support, life skills, and referral sources, and link eligible participants to those events.
  • Identify partners to provide health services and screenings to participants at home or in accessible community settings.
  • Help participants build informal support networks with peers, family, and friends.
  • Engage community or property volunteers when appropriate.
  • Coordinate participant services individually through a person-centered, non-clinical assessment, intervention, and monitoring process.
  • Conduct outreach, educate eligible individuals about the program, offer enrollment, identify needs, and make referrals to community agencies.
  • Conduct telephonic and in-person participant assessments and screenings according to program requirements and established time frames.
  • Collaborate and communicate appropriate information with care and service colleagues to achieve participant goals.
  • Provide customer service by answering questions, addressing concerns, and assisting with basic needs.
  • Help participants obtain and use community services addressing social determinants of health, including housekeeping, meals, transportation, personal services, financial assistance, adult day care, and counseling.
  • Follow up on and monitor services according to participant preferences and contract or program requirements, including home visits when directed.
  • Gather reports, participate in interdisciplinary meetings, and support grants or special programs.
  • Build rapport with participants while maintaining professional boundaries.
  • Manage time independently, complete tasks and documentation within required time frames, and work autonomously and as part of a team.
  • Respond to phone, email, fax, and other communications in a timely and professional manner.
  • Record labor time accurately according to policy and procedure.
  • Complete electronic and written documentation for participant interactions, assessments, and services.
  • Complete required forms for participants using the Service Coordination program.
  • Complete and monitor participant care plans, including follow-up on referrals and services.
  • Submit documentation for funder and quality performance reports.
  • Maintain required caseload levels and complete annual education, compliance, privacy, security, and HIPAA training.
