Requirements
  • Master’s Degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program.
  • Certification from an accredited Nurse Practitioner or Physician Assistant program.
  • Valid Nurse Practitioner furnishing number issued by the California Board of Registered Nurses and Valid California Registered Nurse/ Nurse Practitioner license; or a Physician Assistant License issued by the California Physician Assistant Board.
  • Provider level basic life support (BLS) from AHA approved program.
  • DEA Certification with ability to prescribe MAT.
  • Minimum of 6 months in an ambulatory or community setting.
  • Proficiency in EMR Systems.
  • Demonstrated ability to read, write and speak English fluently.
  • Ability to utilize intermediate/medical Spanish.
  • Proficient in computer programs and systems, including email, internet, and Microsoft Office suite.
  • Knowledgeable in applying evidenced-based medicine to treat and diagnose.
  • Demonstrated ability in applying methods of scientific inquiry and exercising clinical judgment in challenging situations.
  • Commitment to continuous learning and professional development, remaining current on medical practices and professional credentialing and licensure necessary for continuous employment with St. Jude Neighborhood Health Centers.

Responsibilities
  • Responsible for advanced practice in physical and psychosocial assessment, diagnosis and management of health and illness needs.
  • Performing in-clinic procedures at ambulatory-based locations, collaborating with and supporting other providers and caregivers.
  • Assumes responsibility and accountability for the continuity of health care and health education regardless of the presence or absence of disease.
  • Utilizes standardized procedures and consults their Supervising Physician, the CMO and/or Medical Services Manager in cases out of their scope of practice or out of program scope.
  • Documents, via EMR, all treatment protocols and interventions utilized in a timely manner.
  • Services are evidenced-based and tailored to patient population needs.
  • Participates in case consultations, and supports overall continuity of care for health center patients, as needed and guided by department and patient needs.
  • Adheres to department and organizational programs.
  • Upholds all HIPAA and other regulatory compliance and safety requirements.
  • Stays current on all credentialing requirements.
  • Conducts physical assessments, administers or delegates prescribed treatment (including Medication-Assistant Treatment for Opioid Use Disorder), interprets data and dispenses medications.
  • Provides surveillance of medical regimens for the patient and modifies within established procedures.
  • Counsels, teaches and demonstrates care to and for patients, their family members or others who will give care to the patient.
  • Utilizes social worker, nursing staff and other health care team members as appropriate for consultation, referral and follow-up.
  • Provides teaching and counseling aimed at prevention of illness, accidents and disease management.
  • Furnishes input to the development and implementation of standardized procedures, scope of care, clinical policies and new clinical programs.
  • Actively engages in quality improvement goals and activities resulting in appropriate patient care outcomes.
  • Utilizes community resources to optimize patient care and referral.
  • Complies with internal (health stream) and external regulatory compliance and third party payor requirements (for example: Child Health Disability Program (CHDP), Medi-Cal/Comprehensive Perinatal Services Program (CPSP), NCQA, and DHS).
  • Responsible for accurate, complete, and timely documentation and coding/charging for all patient care activities.
