Requirements
  • Experience providing basic primary care and coordinating care with other primary care sites as applicable, including the ability to provide care in mobile and telehealth settings.
  • Experience treating infectious diseases, including hepatitis C and sexually transmitted infections, and prescribing pre-exposure prophylaxis, post-exposure prophylaxis, and doxycycline post-exposure prophylaxis.
  • Experience providing vaccinations, including viral hepatitis A/B, influenza, and human papillomavirus vaccinations, to at-risk populations.
  • Knowledge of medical care for people who use drugs, homeless patients, and people with serious mental illness.
  • A Master’s Degree from an accredited Advanced Nurse Practitioner program or graduation from a Physician Assistant training program accredited by the American Medical Association Council on Medical Education.
  • A Connecticut Advanced Practice Registered Nurse license or eligibility with American Nurses Association certification, or a Connecticut Physician Assistant license or eligibility.
  • Prescriptive privileges or eligibility in Connecticut, a federal Drug Enforcement Administration license, and a Connecticut Department of Consumer Protection license for prescribing controlled substances.
  • Human Research Protection Training is required.
Responsibilities
  • Provide direct primary care and infectious disease treatment in office, mobile health, telehealth, and in-person settings.
  • Provide telehealth and in-person hepatitis C assessments and treatment for the COMPASS project.
  • Provide bridging primary care, sexually transmitted infection diagnosis and treatment, pre-exposure prophylaxis, and other infectious-disease-related services.
  • Provide care for skin and soft-tissue infections and wounds related to injecting, and coordinate care with emergency and inpatient services.
  • Provide overdose prevention, infection prevention, and disease prevention counseling to patients.
  • Participate in team and funder meetings and quality-improvement activities to guide service delivery.
  • Accept referrals from partner sites in Connecticut for telehealth medical management of hepatitis C and other medical needs according to a designated schedule.
  • Complete patient assessments, assess health-related social needs, make clinical decisions, and order laboratory tests and prescriptions for hepatitis C, pre-exposure prophylaxis, and other conditions.
  • Work with Outreach Workers to integrate HIV, hepatitis C, substance-use-disorder, and serious-mental-illness screening protocols into workflow.
  • Examine patients, collect information about their physical condition, and establish clinical diagnoses.
  • Conduct physical examinations, provide treatment, formulate health care plans, and counsel patients on health practices.
  • Encourage treatment-plan compliance through education, assessment of patient understanding, and regular patient monitoring.
  • Provide appropriate medical care, use sound clinical judgment, and practice evidence-based medicine.
  • Obtain, compile, and record patient medical data, including health histories, progress notes, and physical-examination results.
  • Provide preventive health practices to maintain and improve health and prevent illness.
  • Coordinate patient care, including ordering laboratory tests and radiographic studies.
  • Interpret diagnostic test results for deviations from normal.
  • Prescribe therapy or medication as necessary.
Desired Qualifications
  • An Advanced Practice Registered Nurse or Physician Assistant with at least five years of experience.
  • Demonstrated leadership in infectious-disease-oriented clinical settings.
  • Experience working in an integrated clinical setting involving primary care, substance-use-disorder treatment, behavioral health, harm reduction, and research.
  • Training in prescribing medications for opioid use disorder and wound care, if desired.
