Responsibilities
  • Assess, diagnose, plan and implement medical care of patients in collaboration with a physician
  • Accountable for the assessment, coordination delivery and evaluation of nursing care
  • Deliver holistic and individualized care to all patients in assigned area
  • Develop, implement, manage an optimal interdisciplinary plan of care
  • Foster and maintain collaborative relationships between patients, their family/support group, physicians, and other healthcare providers
  • Adhere to hospital policies, industry standards, best practices, and applicable laws/regulations
  • Engage in continuous growth and development in professional advanced nursing practice
  • Follow ambulatory care and inpatient standardized procedures, protocols, policy and procedures regulated by the State of California Nurse Practice Act
Desired Qualifications
  • Experience working with children, adolescents, families, and adults of all ages
  • Experience within primary care settings and in using an integrated care approach
  • Fluency in Spanish
  • Experience using Epic
  • Broad experience in psychopharmacology, the management of psychiatric medications, and the education of patients and their families with respect to pharmacotherapy
